So I'm waiting on an appointment anyone got any guesses
22FTM history of Hypermobile Elhers Danlos syndrome, POTS, ADHD, Patellar kneecap instability, Sleep Apnea, being evaluated for MCAS and TMJ soon. Since years ago I've been having leg shocks, last year it was stronger. I started feeling pain in my back that got worse and worse (I drive doordash for reference) untill I almost passed out from a sudden spike of pain in my back, it felt like I got stabbed, while at a red light (luckily). I was told it was compressed nerves, (they looked at my back physically no machines), I was told to rest it off and start moving again when the pain got better, the pain got a bit better after 3 days, I fell down the stairs, rested for 2 more days, it still hurt a lot, however I am poor and can't work any other job so I went back to driving doordash. I was in physical therapy prior, and went back to physical therapy with exercises they wanted me to do for static nerve pain. Its been getting worse, and worse, Im toe walking more than I did before, which I've been trying to prevent over the years because its a bad habbit. Ive been getting saddle numbness as well with some crossed wires in there, if you don't know there are cases where the brain misinterpreted pain as... Something else. Ive been feeling very awkward about that one, especially when I'm out in public. I went for a walk the other day and my ankle felt stuck for a day after, if I tried to flatten it it felt like pulling on the back of my ankle, like trying to pull a rope with something heavy on the other side, it still hurts when I flatten it, but it also just hurts generally. On top of that my muscles have all been feeling over stressed, my shoulders hurt, my neck hurts, my back hurts, basically everything hurts except my toes lol. I was given lidocane patches that didn't do much if anything. I drank a lot for my partners 21st birthday, and I still felt the pain in my back majorly. Ive been taking 400 mg ibuprofen and 1,000 mg of acetaminophen not doing much of anything. I can't work, I have to ask my partner to help me with upcoming bills, I hate asking for help. Other information When I was in middleschool I started developing numbness in my feet, I also gradually started toe walking. Into high school I literally couldn't feel my feet, I often found scrapes on my feet and legs and one time noticed a trail of blood behind me coming from my foot, from kicking stuff on accident. When COVID hit I slept a lot and I slowly began regaining feeling in my feet, I was toe walking more than ever. When I started working I figured heavy shoes could prevent the toe walking, and it does, when the shoes are on, with that I've adjusted how I'm toe walking so my heels are closer to the ground, my issue is that it hurts when my heels hit the ground. Also I dont know if this is related but I have incidences where I would wake up and my whole body would be tingling like when your leg falls asleep, it was like my skin "fell asleep", the first time this occurred was when I was in elementary school, it goes away after a while, but my skin is just generally sent I've after I wake up. End of other info Im curious if anyone has ever had these issues or if I just sound crazy lol, but seriously I want people to give ideas, Im getting a back MRI this week, but with the extent of it I'm wondering if its even an issue with just my back. submitted by /u/Zealousideal_Care807 to r/DiagnoseMe [link] [comments]

3 years of morning neck and back pain — finally figured out what was causing it and fixed about 90% of it
How long I've had symptoms: About 3 years. Started in my late 30s, I'm 41 now. Came on gradually — wasn't from an injury or accident. Just kept getting worse year over year until it was every single morning. Specific symptoms: Dull ache across my upper back and neck every morning. Stiff neck — some mornings I literally couldn't turn my head to check my blind spot while driving. Occasional tingling down my left arm that scared me enough to get an MRI (came back clean). Shoulder tension that would build through the day, worst by evening. The pain wasn't sharp or sudden — it was this constant low-grade soreness that never fully went away, like I'd slept wrong every single night for years. What made it worse: Sitting at my desk for long stretches (I work remotely, so 8+ hours hunched over a laptop). Sleeping on my stomach. Stress — I'd clench my jaw and tense my shoulders without realizing it. Cold mornings were the worst. And weirdly, the more I tried to "rest" and not move, the worse it got the next day. What made it better: Movement, especially walking. Hot showers on my neck in the morning. The first 30 minutes after waking up were always the worst, then it would loosen up slightly. Ibuprofen took the edge off but I was taking it every morning and that concerned me. How it impacted my life: More than I want to admit. I stopped playing with my kids on the floor because getting up was painful. Stopped going to the gym because I was afraid of making it worse. My sleep quality tanked — I'd wake up 2-3 times a night trying to find a comfortable position. I was irritable from chronic low-level pain. My wife kept telling me to see a doctor and I kept saying "it's just how I sleep." Three years of that. What I was told: My GP said it was "muscular" and prescribed muscle relaxers and told me to stretch more. An orthopedic doc said mild degenerative disc changes in my cervical spine but "nothing surgical" — basically age-related wear. Physical therapist was the most helpful — she said my sleep posture was likely the root cause because the pain was always worst in the morning and gradually improved during the day. Treatments I tried (what worked and what didn't): Here's the full list in order of how much each thing actually mattered: Tier 1 — Changed everything: - Replacing my pillow with an ergonomic contour pillow. My PT specifically said the butterfly-shaped kind that has a deeper contour on the sides for side sleeping and a shallower one in the center for back sleeping. My old pillow was a $15 polyester thing from Target that had gone completely flat. I didn't realize my neck was literally hanging unsupported for 7 hours a night. The contour fills the gap between your shoulder and head so your cervical spine stays neutral. First morning I woke up and didn't reach for ibuprofen. I genuinely didn't believe it would matter this much. - Sleeping on my back or side only — never stomach. Stomach sleeping rotates your neck 90 degrees for hours. I put a body pillow behind me to stop myself from rolling over. Took about a week to break the habit. Night and day difference. - Getting a new mattress. I had a memory foam mattress that was 8 years old with a visible body imprint. Your spine can't align on a surface that sags in the middle. Switched to a medium-firm hybrid. Not cheap but the old one was literally curving my spine while I slept. Tier 2 — Noticeable improvement: - PT exercises before bed — 5 minutes. Chin tucks (10 reps), slow neck rotations, and doorway chest stretches. The chest stretch surprised me — if your pecs are tight from sitting all day, they pull your shoulders forward which strains the back of your neck all night. My PT said most neck pain patients have tight chest muscles. - Adjusting my work setup. Got a $25 laptop stand and external keyboard so my screen is at eye level. I was looking down 8 hours a day. The neck pain during work dropped by half, which meant less accumulated strain going into sleep. - Magnesium glycinate before bed (400mg). My muscles actually relaxed more at night. Less jaw clenching, less shoulder tension. Better sleep quality overall. - Walking 15-20 min after dinner. My PT said it downregulates the nervous system and helps your muscles release tension before sleep. On the nights I skip it, I notice the difference next morning. Tier 3 — Helped but slower to notice: - Hydration. Your spinal discs are mostly water — when you're dehydrated they compress more overnight. I went from maybe 4 glasses a day to about 3 liters. Took 2 weeks but the morning stiffness got noticeably better. - Anti-inflammatory changes — omega-3s daily (fish oil + salmon 2x/week), cutting way back on sugar, turmeric in my morning coffee. Took 3-4 weeks but my baseline inflammation dropped. My fingers stopped being puffy in the mornings which I didn't even connect to inflammation until it stopped. - Upper back strengthening 2x/week. Rows, face pulls, band pull-aparts. Building the muscles that support your spine means they hold everything in place better while you sleep. Took about a month to feel the difference but it keeps compounding. What I wasted money on: - Chiropractor 3x/week for 4 months ($2,400 total). Felt amazing for 2 hours after each visit, then right back to square one by the next morning. My PT explained why — my sleep posture was undoing every adjustment overnight. Expensive temporary relief. - Cervical traction device from Amazon ($40). Uncomfortable, used it for 2 weeks then it sat in a drawer. - An expensive memory foam pillow from a big brand ($180). Way too soft — my head sank in and my neck was still unsupported. Learned that expensive doesn't mean supportive. The shape matters more than the price. - Pain relief creams, patches, Tiger Balm. Masking symptoms. The pain was mechanical — from my spine being misaligned for 7 hours every night — not something topical could fix. - Muscle relaxers (prescribed). Made me groggy, didn't address the cause, and I didn't want to take them long-term. Where I'm at now: The pillow and sleep position changes fixed about 60% of the problem within the first week. The stretches and work posture fixed another 25%. The diet, hydration, and strength training got the last 15%. I'm not 100% pain-free — long flights still get me, and if I sleep in a hotel with a flat pillow I feel it immediately the next morning. But going from daily pain and 3 ibuprofen a day to occasional stiffness maybe once a week is life-changing. If I could go back and start over with just $50 and one week, I'd get a proper contour pillow and do the PT stretches. That alone would fix it for most people. The rest is optimization. What's worked for everyone else? Curious if the pillow switch was as dramatic for anyone else or if my old setup was just exceptionally terrible. Also wondering if anyone else's pain turned out to be more sleep posture than actual spine issues — feels like that gets overlooked a lot. If anyone wants to know which one, happy to share over DM. submitted by /u/PhysicsObjective6875 to r/ThisActuallyWorked [link] [comments]

Been sick for a month I need management tips
Hey everyone so I’ve been sick for just about a month, urgent care said it was an ear infection and gave me antibiotics but my throat is still on fire no matter what I do. I’ve tried ibuprofen, tea, soup, steaming up the bathroom, warm AND cold compress on my neck, CBD lotion, etc etc. I will be going back to urgent care tomorrow night but I need to go to work tomorrow (I have missed way too much from being sick even with the doctors note) and I need any sort of tips on temporary throat relief so that I’m able to talk to customers before I go to urgent care. Also I would like to be able to actually sleep tonight without being distracted by the pain. Any tips that I haven’t already tried are greatly appreciated! submitted by /u/doublesnarkle to r/sick [link] [comments]

Hiking 45.6 miles in one day (pa michaux Appalachian)
Hello, (see edit at bottom for changes to the idea from your lovely peoples advice) I am going to be graduating college soon. I was in horrible accident 22 months ago, I had 4 spinal fractures and many other broken bones and I want to hike the 45.6 miles in one day as my celebration of life hike. Ive done 26 miles (1800 elevation) in one day before as my last celebration of life hike. I have played all day in sports tournaments. I have ran 10ks and one half marathon. I would try to hold 3 mile+ on flats and have held 2.6 on uphills when hiking through the maroon bell passes. I am very farmiliar with michaux and have hiked around 400 miles of its trails. Tuesday is showing 55 at 4am, a 78 non sunny high and 64 at midnight. Its about 6000 feet of elevation and I have a loving girlfriend willing to drop me at 4am, meet me at the midpoint for a restock on baltimore road, and pick me up around midnight. I have done a few night hikes before but not that level of mileage at night. Im not afraid of bears or coyotes as I have already encountered them at night and successfully scared them off. I know I would need around 30 lbs of liquids or more. I dont have any fancy hiking gear, would be an eddie Bauer backpack. -10 32 oz body armors, probalbly half of the lite -108 oz of water -massive flashlight (500 foot range) and spare battery rails -bear spray -handy dandy talon knife -bandaids for blisters -spare undies and socks -some wipes for the trails and a baggie for the used ones -tums and ibuprofen -lightweight hoodie for the morning -compression shirt and shorts under neath -reverse taping my socks because lonestar ticks scare me the most out of any wildlife -portable charger for phone and flashlight rails Looking for advice, from people who have done similar milage in a day. Also taking tips on the packing list. edit with the helpful advice from you all 2 aid stations, going to stay around 4 32 oz bottles and 32 oz of water at a time. Im planning for 8000 calories. I will start much earlier towards 2am or midnight to not be hiking the end at night. Im keeping the flashlight, it calms me, and is also a billy club. But i will turn it over at the first aid station. I will be brining some cornstarch for the chafing. I love you all and I love pine trees. ill ditch the portable and low power mode the phone. submitted by /u/parkerbjorn to r/hiking [link] [comments]

needing advice for severe pain
hi there! I just got double incision top surgery 3 days ago. it was my first surgery so I knew that the pain levels would be different than what I'm usually used to even though I have a very high pain tolerance. when I woke up from surgery I could immediately feel that my left drain site was really painful and more swollen compared to my right side but I figured it was normal and went home. I've been taking all my pain meds (5 mg oxycodone, 1000 mg tylenol and 400 mg advil) as recommended on a rotating schedule, which has been helping but not fully alleviating it. I also take gabapentin at nighttime which is supposed to help with nerve pain but unfortunately has not been working for me in this case. before today, it only flared up if I moved a certain way but unfortunately it's only gotten worse and this morning I woke up in 9 out of 10 red hot burning pain all along my entire left side and I could barely move. I was shaking and nauseous, but thankfully I don't think I had a fever. I panicked and called the nurse line for the hospital I had surgery at. it took the whole day to get it sorted and they asked me to send over pictures of my incision and drain sites which I did. as far as anyone can tell nothing is infected! it all looks irritated but I'm only 3 days post-op so that's to be expected. I packed on some extra gauze before I put my compression vest back on which helped a bit, and I felt better after taking my pain meds. my drains are working properly, though my left side is draining significantly less than my right side (19 ml vs 35 ml tonight) so idk what that means. I'm taking it easy and not over-exerting myself, and my incisions look fine, I'm just in an extreme and unexplainable amount of pain. I'm supposed to walk around and do stuff but the amount of pain I'm in is really preventing me from doing that :( I'm posting here mainly just because I'm really nervous and curious about other people's experiences. the oxy only helps a bit and I really don't want to become dependent on it, but also the ibuprofen and advil aren't touching it so I kind of have no choice. they told me to go to the ER if it gets worse but I can't really afford to and I don't know if it would even help! one of the doctors mentioned lidocaine patches but I don't know if that would be safe to put directly onto my incisions or not. has anyone else had to deal with severe pain like this during their recovery? if so what did you do? do I really need to go to the ER? am I doing something wrong? I'm thinking I might just need to keep taking the oxy and making sure to change my bandages a lot. I'm just worried that something is wrong that I'm not paying attention to. they told me not to use ice or heat so I think I'm doing what I'm supposed to be doing. just stressed out and would appreciate some insight from someone else. thank you in advance! submitted by /u/glassbrigades to r/TopSurgery [link] [comments]

In-depth MA experience
Sharing my experience (21F and 7w5d pregnant with low blood) reminder that experiences may vary for each person, but I found reading posts like this during my preparation very helpful to ease my fear and anxiety. When I found out I was pregnant, I immediately knew where to look for resources— Reddit. I read posts in this subreddit nonstop until I felt I had enough knowledge to proceed. TVS experience: Transvaginal Ultrasound (TVS) is a type of ultrasound where a medical apparatus (probe/wand) will be inserted in your vagina to check into your uterus. At first, I was scared because It was my first time. But it was quick and painless. just a bit uncomfortable since I was ‘squeezing’ the wand because I was nervous. I just told them that I wanted to confirm my pregnancy since I did my PT and saw a faint line. There were not much questions asked aside from my LMP. if you’re worried with this procedure, don’t worry because it usually lasts for up to 5-10mins only. fast and easy. I prepared this ahead of time prior to contacting Fp** because they require a TVS result with Age of Gestation (AOG) at around 6wks and above. Fp** transaction: Once I got Fp**’s contact, I messaged them immediately with my TVS result. (they require to see the whole result, with your whole name and name of the clinic). Surprisingly, they were quick to response. after sending my payment, they immediately shipped out the meds and I received them the next day. The pills were 4,000 + shipping fee. mine was 300 and I’m around Metro Manila. The pills will come in a ziplock inside a contraceptive pills box (1 mife, 8 miso)but rest assured that it’s legit since it’s from Fp** they are very cautious and discreet with the transaction. although they know you’ll buy abortion pills, PLEASE do not mention that word upon interacting with them. be as discreet as possible and understand that this is a VERY risky transaction with legal consequences if compromised. Things I prepared: * Period pants (charmee, 6pcs) * Tissues and Wipes * Extra Towels and Bed covers * Extra Shorts and Underwear * Thick Blanket * Hot Compress (electric) - such a live saver, I suggest you have one with you. Medicines: * Bonamine for Adult (25mg) * 4 Medicol Advance (400 mg) * Antihistamine (in case of allergic reaction) * Iron Supplements Food/ Others: * Pocari Sweat * Biscuits * Soup * Water * Bread ————————————————————————— The process: [April 3] 🕛 8:32pm : Took Mife - Felt side effects around 3am; light cramps and chills - Also had a fever the next day [April 4] 🕛 8:30 pm : - took Bonamine 🕛 9:30pm : - took 2 ibuprofen (400mg) 🕛 10:00pm : Miso 1st Dose - At 25mins, I already swallowed the remnants and when I did, I immediately vomited - I was anxious since I wasn’t able to complete 30mins and I saw that my vomit had the remnants of the pill Tip: Sit upright, Keep your head straight or upwards. As much as possible, do not think that you have meds under your tongue, you’ll forget about it eventually since the meds don’t have any taste. I think swallowing your saliva during this time was the main factor why I vomited so try not to swallow your saliva at all. 🕛 10:30pm : Cramping began - Shortly after swallowing the pill, I began having cramps. It came in waves like contractions - The pain was bearable, the hot compress and pain meds helped a lot (7/10 pain) - Although the pain was lessened, I was still shaking esp. during the ‘contractions’ 🕛 10:30-11:30pm : - I just stayed in the restroom, although there was not much blood coming out, it was mostly diarrhea and contractions at this moment 🕛 11:40pm : - decided to lay in bed, pain was 5/10 and I just wanted to sleep [April 5] 🕛 12:50am : - Woke up and felt the cramps again, I immediately went to the restroom since I felt that something would come out - Expelled the Sac at 12:55pm - Make sure to rinse whatever you expel and take photos of it. you will need this for confirmation (to send to fpop or other people who can verify) 🕛 1:00am : - Drank another 400mg of Ibuprofen in preparation for the 2nd Miso dose 🕛 1:30-2:00am : Miso 2nd dose - Took the 2nd dose; this time I was able to complete the 30mins and did not vomit after swallowing - Did not feel cramps or anything after swallowing so I fell asleep 🕛 3:10am : - Woke up due to cramps, it was more bearable than the one during the 1st dose (5/10 pain) - Went to the restroom and started expelling blood clots - Blood was dripping everywhere and I decided to squat on the floor since I think the placenta is hanging off my vagina 🕛 3:12am : - I was just squatting and I felt like I was about to poop then suddenly the placenta came out - I rinsed it then took photos/videos for confirmation by this time, I wasn’t feeling any pain. although I felt weak due to the entire procedure itself. It was tiring and draining. My companion cleaned up and i immediately went back to bed after cleaning myself up. ——————————————————————————————— If you have low blood like me and will undergo this procedure, please prepare ahead of time. drink iron supplements and eat healthy. I wasn’t able to fully prepare beforehand and I felt series of ‘blacking out’ through out. Right after my procedure, I drank my vitamins and drank a lot of pocari and water all through out. That was everything that happened, while writing this, I can’t imagine how I went through all those nights full of worries if I would be able to do it. but now, It has passed by, just as everything does. and I’m glad I went through with it. It was the right decision for me and my partner. I just hope that we have a lot more options for basic healthcare like this in our country. Abortion doesn’t have to be scary and done in the shadows; it should be a basic human right for every woman. If you’re planning to do this and you’re scared and feel alone, I believe in every woman’s strength, we go through so many changes within our bodies throughout our lives; we experience pain monthly, we are the ones bearing children and the ones dealing with all the pain and recovery. Women are so physically and mentally strong to be able to go through all that. So think of that, think of how strong you really are; for choosing this and for making such big decision— not just for yourself, but for your soon to be angel. I know you’re doing what’s right for you. it’s your right to decide for your body and for your future. I also want to thank some people out here who helped me and talked to me from my preparation and during my procedure. I’m forever grateful to all of you. 🤍 currently, I’m still recovering and upon confirming with Fp\*, they said that my* MA was successful submitted by /u/unevenstar09 to r/Abortion_Philippines [link] [comments]

12 weeks FPOP
things we prepared: - kotex freedom pads 32cm - pocari sweat 2L - ibuprofen - snacks - small carton with plastic na nakabalot in case magka tagos - hot water in a small jar hot compress para sa cramps - maliit na planggana - seamless undies 6 pcs - 3 shorts - socks monday - sent the tvs. 12 wk, 0 days na ako. tuesday - still no replies. nag chat ako kung avail ba ung meds. nag send sila ng payment details. wednesday - paid the amount thursday - nag send sila ng instructions friday - as soon as dumating yung package, we decided to go thru it. around 5pm ko ito na receive. 6:24 pm - i took the mife wala akong na feel na something hanggang kinabukasan. so far parang usual na araw ko lang. wala akong ginawa to prep my body. basta rest at nap lang pag inaantok or pagtapos kumain. 24hr has passed. saturday, 7:30 pm - i took ibuprofen 400mg kumain muna ako ng dinner. hotdogs at chicken nuggets. kumain din ako ng moby as my snacks. 10:12 pm - i put 4 miso under the tongue for 30mins at this point, i was waiting sa reply ni whw dahil i’m getting confused kung anong susundin ko na procedure kaya nagpa consult ako via email. magkaiba kasi ang alam ko na dapat gawin doon sa instructions ni s2c. from s2c: take 1 mife swallow 4 miso sublingual 4 miso sublingual from whw: take 1 mife swallow take 2 miso sublingual after 3 hrs, 2 miso again after 3 hrs, 2 miso again after 3 hrs, 2 miso again until it is completed hindi agad lumabas sa notif ko yung response nila so sinunod ko yung instructions ni s2c. after 30mins i swallowed the remnants of it, maya maya nag suka ako. lahat ng laman ng tyan ko nilabas ko. na irritate. i realized na dapat pala hindi ako kumain ng mabigat sa tyan. dapat light lang. dry food like biscuits para hindi masyado liquid. kasi ang bagal pala matunaw ng hotdogs at chicken nuggets kasi buo buo pa sya sa suka ko. never na sa too processed foods. minutes after, nagutom agad ako. kinain ko yung cup noodles kasi sabi ko konti lang naman yun tsaka may soup din. after that nagpahinga na ako. around 11 pm - i started getting chills. 1hr and 30mins na yung nakakalipas. gets na umeepekto na yung miso. nagsuot na ako nito ng socks. pina turn off ko din ung e-fan. 12 am - nawala din yung chills. pero nag start na yung mild cramps. nag hot compress ako while watching the series ng adventure time. maya maya inantok ako so nag nap ako saglit. nag set ako ng alarm ng 1:12 am. ginising ako pero parang ayoko pa. sobrang sakit kasi ng ulo ko. so tinuloy ko yung nap ko until 1:45 am. 1:47 am - took 2 miso sublingual since na irritate nga yung stomach ko sa ginawa kong 4 pills ng miso. i decided na sundin na lang yung kay whw. kaya 2 miso na lang muna. 2 am forward - ramdam ko na yung cramps. moderate. sinabayan pa ng headache. so hilo talaga. sinuka ko yung kinain kong noodles. walang natunaw. same sya sa kung paano ko nginuya wtf. nag change rin me ng pads at this time. daming blood clots. 3 to 4am - i took 2 ibuprofen 400 mg ulit. around 4 am - i change my pads again. masakit na yung cramps. i can’t stand too long. hindi ko na rin nainom yung last 2 pills kasi hindi ko na kaya. 4:50 am yung alarm ko pero hindi ako makabangon. around 5 am - naiiyak na ako sa sakit. pina massage ko yung belly area using tiger balm. hindi na ako makatulog ng maayos. sobrang sakit na talaga like as in sobrang sakit. kahit anong baliktad ko. no effect. i relaxed a bit when i hit the child pose. 5:45 am - bigla akong nagising sa nap ko at napatayo nung may na feel ako na parang may biglang pumutok around my butt. naging sobrang lakas ng flow ko. natagusan ako buti na lang nasa ilalim ko yung carton with plastic, hindi sya kumalat sa foam ng higaan. yung blood may kasamang water. so ayun getssss i guess my water broke. i expected na malapit na sya lumabas. iniisip ko this time hindi ko na nainom yung last 2 pills. sabi naman sa instructions ni whw, 6-9 hrs nagtetake effect ang miso after taking the first dose. so i trusted the whole process. dali dali akong nagchange ng pads at shorts. after ko magpalit, feel ko parang may lalabas ulit. so nakatayo lang ako hanggang mag 6 am para hindi ako matagusan at magkalat huhu. 6:09 am - lumabas na sya 🥹 nagsquat ako sa naka ready na small planggana. nag squat lang ako. hindi ko hinila. wag daw hihilahin kasi kung naka sabit pa sya, it means nasa loob pa yung placenta. so i waited. nagpa help ako na ilabas yun by stimulating my tatas. after a few mins, naging ok na. hayyy. at 12wks, kita na pala kung girl or boy ba 🥹 it’s a boy. it feels nice kasi i guess the time i conceived him, i wasn’t stressed. puro kain lang din ako and watching series to make me feel better kasi a month and a month before that, it was super challenging. dami kong iniyak nun. pero it was nice to know na i did my self care very successfully 😂 to the point na my body felt safe to conceive but ayun habang tinititigan ko sya, i forgave us. siguro pag kaya na namin sya bigyan ng promising life. we’re still building ours eh. ayaw din namin magpasa ng traumas sa kanya. financially. mentally. emotionally. they deserve the better version of us. anyways, to the person who is reading this, i hope u find this helpful. patuloy na progress pa sana para sa karapatan ng mga kababaihan. happy national women’s month! submitted by /u/Xoxoberries to r/Abortion_Philippines [link] [comments]

Commonly asked questions (BEFORE, DURING & AFTER MA)
BEFORE MA Where to buy legit pills? - you downloaded reddit, please utilize it in researching about source of legit pills here in the PH. Ilan pills po usually ang need or binibigay if legit? - 1 MIFEpristone (mas malaki kesa sa MISOprostol) + 8 MISOprostol (4-9 weeks of pregnancy) or 12 MISOprostol (9-13 weeks of pregnancy) - P.S if more than this ang binibigay ng seller most likely fake ang MIFE and counterfeit ang MISO Are there any preparations needed before doing the MA? - Nothing, just rest, hydrate and eat healthy food before doing para may lakas ka during the procedure. Need po ba uminom ng ibuprofen or bonamin before mag MIFEpristone? - Usually no kasi wala naman side effects usually ang MIFE kaya no need pero if may pain kayo nararamdaman ayos lang uminom ng ibuprofen. Ano po ang mga need during the procedure? - Ibuprofen (2 pcs 400 mg or 4 pcs 200 mg) - Bonamin - Menstrual Diaper - Tissue - Hot compress - Electrolytes (Pocari sweat or Gatorade) - Water - Snacks DURING MA Ang MIFEpristone po ba ay nilulunok agad o kailangan din ibabad ng 30 mins under the tongue? - Yes, kahit anong inumin at kainin mo during the procedure ay pwede. - No, pero it depends on you if I will to ask I will still take it to be sure na lumabas lahat ng dapat lumabas Ano po ang need gawin para lumabas agad yung POC? - Maglakad lakad after inumin ang MIFEpristone at after din ng first dose ng MISOprostol, during MISO you can squat, jump and walk around para lumabas siya especially if pag nag cracrams pero if kaya lang ha don't over do yourself. Paano po malalaman if lumabas na siya? - If hindi mo siya nakita there's really a sign, if from having a heavy flow and pain cramps is mag subside siya and maging moderate to slow bleeding nalang then cramps will be from 10/10 maging 2-4/10 ganon. Tapos parang ngalay yung pelvic or balakang mo pero no that painful naman. - Pero usually tissue siya na kakaiba from blood clots na nadudurog para siyang spongy or liver-like appearance then makakakita ka ng greyish tissue na naka attach sakanya. Minsan grey siya or clear siya then kasabay non yung liver-like texture, pero iba iba naman yung nilalabas natin and it depends on the weeks of pregnancy pero if hindi ka sure if nalabas mo na usually yung first statement ang sign. After the procedure ano po ang mag need and hindi dapat gawin? - Rest lang and don't exhaust yourself, stay hydrated and get a lot of rest na need mo pero you do the things that you normally do naman perp if hindi kaya wag pilitin. Ano po ang signs na failed ka or may problem sayo? - Foul discharge, hindi bumababa ang lagnat for more than 24 hrs, severe cramping for more than 24 hours, soaking of 2 pads or more per hour. AFTER Need po ba magpa-tvs after the procedure? - No, As long as there are no complications and negative na ang PT 2-3 weeks after MA. Pero if want mo talaga to make sure at 2 weeks pwede na Hindi po ba madedetect na nag MA if magpapa tvs? - No po, as long as hindi ka nag insert vaginally hindi madedetect na nag MA ka. Ilang weeks po usually pwede na mag PT? - 2 weeks onwards pwede na pero if positive padin ang PT it is still normal check again at 3rd week and 4th week. Normal lang po ba na on and off ang bleeding and ang spotting? - Yes, normal lang po as long as there is no foul discharge and hindi super heavy ang bleeding to the point na you're soaking 2 or more pads per hour. KAYA NEED MO IMONITOR ANG BLEEDING MO AFTER MA. These are the commonly asked questions sa akin, pero if you guys have other questions you can ask others here din who undergone MA. I hope this helps guys! For the procedure of MA, you can come to my profile and read my first post here, you can also see my MA journey during and after! submitted by /u/Powerful-Garbage-967 to r/Abortion_Philippines [link] [comments]

RTW - Indefinite (?)
Hello, 16L Setup: https://lighterpack.com/r/r5ynlp -------- Edit (01/29) - quick unpacking video of the 16L bag: https://youtu.be/7airAPO2n_M Edit 2 (02/04) - I made a few changes, bringing the bag down to +- 5500 gr while getting more functionalities. -------- There are a few things I can surely improve (the pants on me, add merino gloves, swimwear....). But overall, it doesn't sounds that bad for now. A lot of tech in the list. I'm working online. Also doing volunteer work time to time so I'm able to provide photography/videography (ex: in a dog's shelter, making epic contents of them so people want to adopt them). It allows me to be in any environment from -10°C to 35°C (14°F to 95°F). I like the cold and I'm ok with the heat. I always wear pants (2 years in Thailand wearing it) so I don't feel the need for shorts. It feels comfortable to have the bag full time on me as light for me (185cm/95kg). If needed, I can let the bag at hotel and still having the pouch on me. If you have any suggestions or remarks, it's welcomed. ma experience

1 week since FUT 2030 grafts with Dr. Carman (plus Post-Op Care instructions)
I am sharing my experience with my recent FUT transplant with Dr. Timothy Carman at the La Jolla Hair Restoration Medical Center in the San Diego, CA area. My transplant was 2,030 grafts: 1,200 on the front hairline, 830 on the crown. My photos include right before the procedure, 1 day post-op, 7 days post-op, and the last photo is 12 days post-op. At the bottom of my review, I have included the text of the full Post-Op procedure list provided by La Jolla Hair Restoration Medical Center for reference for anyone who wants to know what to expect and all precautions you will have to follow up to a month after the procedure. Pre-Op: I live in the San Diego, CA area, and have a Norwood 3-Vertex pattern with a shiny-bald crown. I've had a receding hairline and crown starting at age 20. I’ve taken 1 mg finasteride daily for 5 years and have been inconsistently applying topical minoxidil. I intend to get back in the saddle for minoxidil after 30 days post-op. My hairloss has slowed but still progresses at a rate of approx. 200 grafts per year, estimated by Dr. Carman’s consultation estimate changing from recommending 1,400 grafts to 1,800 grafts after 2 years between my first and last consultations and from comparing photos. I paid $11,000 for 2,030 grafts, including a $1,500 deposit to reserve the appointment. Average charge: $5.47 per graft: Dr. Carman’s graft cost calculator: First 1200 grafts cost $8,600. All additional grafts are $3 each. Donor strip incision length is 20cm (7.87 inches) and 1 cm wide (.39 inches). I had two in-person consultations plus a phone consultation leading up to my appointment, all free. Dr. Carman has done hair transplants at La Jolla Hair Restoration Center since 2003 and does FUT exclusively. He advocates hard for it with his biggest point being the higher viability of grafts during the excision process with lower likelihood of damage. He is an intriguing cheerful soul who makes great conversation and calls himself a sculptor using his patients’ scalps as canvases and their hair as paint. I trust him and believe he has the expertise to minimize the width of my donor scar and place the new graft locations in the most natural way he can with each hair facing the direction of my natural hair pattern and cowlick on the crown. My new hairline is conservative and mostly reinforces the thinning portions of my existing front hairline due to Dr. Carman’s inspection of my donor area and taking into consideration my lifetime donor supply with average density in the middle of my head and less-than average density on the sides. I’m happy with the location of my 31-year old hairline and consider I will still be happy with it when I turn 70 years old. The density of my crown transplant is also thinner (830 grafts) due to Dr. Carman’s insistence that the rest of my future donor grafts would be used much more purposefully on my mid-scalp and frontal region as my hairloss progresses potentially to Norwood 6 or 7. During his consultations, he ensured I keep my vision on the future instead of filling my crown to the same density as the rest of my head and bringing my front hairline down to where it was in my early 20s. The Procedure: The procedure took about 11.25 hours from the first lidocaine shot at 7:45 am to getting out of the chair at 7:00 pm. I’m glad I was allowed to keep my hair long for the procedure. I was given antibiotics and a dose of Xanax right before starting. I sat back in a reclining chair with no headrest. Dr. Carman shot me up with about 20 lidocaine shots orbiting around my head using a vibrating massager next to each shot to overwhelm and distract the touch-sensing nerves. Pain level was fortunately not as bad as I thought it would be. The level of pain was equivalent to getting my eyebrows threaded. After a minute, I was all numbed up and didn’t feel a thing though I did hear my skin graft get cut in the back. About halfway into the procedure around hour 6, I started to feel a loss of numbness and alerted the techs right away to ask Dr. Carman to inject me with more lidocaine. Dr. Carman cut my donor skin graft out of my head, sutured the donor scar, and placed micro-incisions on my hairline and crown with a scalpel, making different sized incisions depending on whether I needed a single, double, or triple graft implant. Dr. Carman works with three friendly techs, who were also nice to chat with to pass the time of my 11 hour procedure. The techs were responsible for removing and prepping the grafts from the donor skin graft and then implanted them into the “fence post” micro-incisions placed by Dr. Carman. The techs differentiated the incisions meant for a single graft, double graft, or triple graft, depending on the size of the micro-incision Dr. Carman made. Post-Op: After the procedure, I took antibiotics twice a day and Vicodin and 400 mg ibuprofen for pain every 4 hours for about 3 days. I got my hair washed during the next-day follow-up appointment. When showering, I was only allowed to lightly pat my head with baby shampoo. I was able to sleep for 7 hours each night on average despite the discomfort of sleeping at an elevated angle and feeling uncomfortable from my neck pillow putting pressure on my sutures while I slept. I had to sleep this way for four nights. As the days passed, dull pain changed to itchiness around the donor area as scabs formed and cracked. I took Benadryl and was able to resist scratching. I was able to go a full day without using any painkillers or Benadryl by Day 8. The worst effect of my surgery by Day 12 is the ugliness of the scabs flaking off and getting stuck in my hair, even with shampoo (see last photo). On Day 8, I got my sutures removed in about 30 minutes. Suture removal was surprisingly the most uncomfortable and sometimes painful procedure of the entire transplant process, even when using a vibrating massager near the scar to help overstimulate the touch sensors of my skin. The pulling sensation on the back of my head would sometimes sting like being jabbed with a needle, so it was done in phases with short breaks. I didn’t feel any soreness a couple minutes after the removal was over. I will give updates again in a couple months as my transplant progresses. Even being a year out from my final results, I highly recommend getting a consultation with Dr. Carman who will take care in doing responsible conservative transplants with a natural-looking hairline, so long as you’re okay with getting an FUT. I have copied all the post-operation procedure documents I received from Dr. Carman below as a reference for future transplant patients to be informed on what to expect post-transplant: __________________________________________________________________________________________ LA JOLLA HAIR RESTORATION MEDICAL CENTER POST OPERATIVE INSTRUCTIONS ~ Your First Night ~ - Please take it easy after your procedure. Relax, rest at home, read, watch TV, and have a nice meal (you will probably fall asleep anyway - enjoy the rest). - Anticipate that there should be some pain, especially in your donor area. It is recommended that you consider taking at least one Hydrocodone /APAP (your pain medication) every four hours, regardless of whether you feel pain. There may be a little bleeding into your bandage or onto your pillow. This is normal, so don't be alarmed. There are a few basic principles that help to remember our post operative instructions: -The first 4 days following surgery you should sleep on your back so that your head and back are at about a 45 degree angle with the horizontal. This can be accomplished using pillows in a bed or by sleeping in a "Lazy-Boy" type recliner chair. -If propped up on pillows in bed, make sure you are supported on either side so that you don't roll flat onto your side during the night. -Any unnecessary heat to the scalp may result in significant post-operative swelling; the following should be observed: -Don't stand in the hot sun. -Don't heat your body with exercise. Anything that causes you to sweat the first week should be avoided. Follow this advice for five days or until any swelling has disappeared. . If you have swelling of the forehead, do not be alarmed, it only affects your appearance over the first week. It does not affect the outcome of your procedure. · It is okay to restart aspirin, vitamin E, and foods rich in garlic 10 days after your procedure, unless directed otherwise. (Your blood has clotted as much as it needs to at this point.) Since alcohol dilates blood vessels, it would be advisable to avoid alcohol use until the risk of swelling has abated. (Usually 5-7 days.) · You will have a bandana and bandage in place following surgery, please do not remove them. Sleep with them on, and we will remove them during your first post-op visit tomorrow. If the bandage rises, keep it in place by gently moving it towards its original position. If it falls off, do not attempt to replace it. If you feel you have a Medical Emergency tonight following your surgery, CALL 911 !! Otherwise, if you have any routine questions that cannot wait until tomorrow, please telephone Dr. Carman. In the unlikely event that you cannot reach anyone, and you have a non-emergent situation, of course consider seeking help at the nearest open medical facility. ~ Your First Week ~ -Follow this advice for five days or until the swelling has disappeared., if present -Swelling of the forehead, should it occur, only affects your appearance over the first week. It does not affect the outcome of your procedure. Applying a package of frozen peas or crushed ice to the forehead (but NOT to the grafted areas) may be helpful in reducing swelling by taking heat out of the scalp. -The scabs fall off by themselves between 7 and 12 days. As the scabs come off, you may see a hair with them. When this is the case, you are not losing one of your grafts. The follicle is merely going into its resting phase and is letting go of its fiber. Its growth will restart in about 3-4 months. - Some numbness of the recipient and donor areas is not uncommon. The recipient area recovers its sensation over several months; the donor area numbness, when present, can take a longer period of time -Transplant "shock" occasionally occurs to pre-existing hair which is present in areas which in we transplanted grafts. This refers to the temporary loss of the pre-existing hair in these areas as a result of localized swelling and inflammation. Just like all of the grafted follicular units will go into shock and fall out, these pre-existing hairs may follow that same process. Just as with the transplanted hair, they grow from the resting phase over the next 3 to 12 months -The growth of the grafts usually starts between month three and four. At six months you generally have about 50- 70% of the grafts "sprouted". At about 14 months post surgery, you should have a pretty good idea of what has been accomplished, as 95% should be grown and matured by this point. Shampooing your hair: -Starting on post-op day 2 (the day after your hair wash with us) as we showed you, pouring water on your head, then patting the area with Johnsons Baby Shampoo, then rinsing well. Avoid medicated shampoos (e.g., Head and Shoulders) -Again, be careful with how forcefully the water hits your grafts for the first several days. We recommend that you use a separate container such as a large cup to catch the water and then pour it on your head, as opposed to letting the shower stream hit directly on the grafts. -A gentle "patting" or "dabbing" on the grafted area with your fingertips is all that is required; avoid circular motion or rubbing with your fingertips for the first week. ~ Post Procedure Information ~ Scabs You will have scabs for about 7-14 days. As the scabs fall off, the hair shaft will often come out with them. This is normal. The follicle is still lying beneath the skin. And remember, it will take 3 to 4 months before you should expect to see any hair growth. Numbness You may experience some numbness in both the recipient area and the donor area. This is normal. Small nerves have been severed and are mending back together. The numbness will gradually dissipate, but can be perceptible in the recipient area for up to 6-8 months and in the donor area for up to one year. Redness You may experience redness on the transplanted area for 3-6 weeks and possibly for a longer period of time if you have very light skin coloration. This is a normal manifestation of the healing process taking place in the grafted areas. Pimples It is not uncommon for patients to have several pimples appear on the grafted and donor areas as the resting hair begins its growth phase. We would like you to treat them by applying a warm compress to the area 3 times a day for ten minutes each time. Do not squeeze as this can make the situation worse. If the pimple is not gone in 3 days please give us a call. Hiccups Some patients may get hiccups. This is caused by an irritation of the skin nerve in the donor area. Raising the CO2 level in the blood by breathing into a paper bag for a few minutes usually stops the hiccups. An alternative solution is to allow a teaspoon of sugar to slowly melt under your tongue while you hold your breath for a comfortable period of time. Sunscreen It is advisable to limit your sun exposure for 6 months after your procedure. The sun may cause the grafted area to lose or gain pigmentation. You can wear a hat or put sunscreen (sunscreen after 30 days post op) on the grafted area. Exercise You may resume low level exercising such as walking, seven days after your procedure. You should wait four weeks before you resume high impact or vigorous sports, including water sports. High impact or vigorous sports earlier than four weeks may result in greater scarring of the donor site. Hair coloring We ask that you wait for 30 days after your procedure before using any hair coloring products or heavy alcohol based hair products. ~ POST PROCEDURE MEDICATIONS ~ -Antibiotic: Cephalexin 500mg Take one capsule twice daily until all capsules are gone. -Pain medication: Hydrocodone W/APAP 5/325mg (Vicodin) Take one or two tablets every 4 hours as needed for pain. Please take this medication with food and water. Do not use in potentially hazardous situations, such as driving or operating heavy machinery. -Pain medication: Ibuprofen 200mg Ibuprofen works well to control the post-operative pain, especially in those individuals who do not want to take, or cannot tolerate Vicodin. Each over the counter tablet is typically 200mg, and you can take 1-3 of the 200 mg tablets every 6 hours for pain. -Prednisone: 20mg Take 1 tablet twice a day with food until all tablets are gone. This medication helps to prevent forehead swelling and inflammation. -Facial/Extreme forehead swelling medication: Furosemide (Lasix) 20mg Do not take this unless you see exaggerated signs of swelling. Most will not experience this. At the first sign of increased forehead swelling, take 2 tablets at 9 AM and again at 3 PM. Repeat this each day until swelling is gone. This type of unusual swelling may not start until day 3 or 4 after your surgery. - Minoxidil 2% or 5% (Topical) Minoxidil 2.5 mg (Oral) We recommend waiting until about 30 days post-op to commence using minoxidil. This ensures that all areas have healed sufficiently. Resume oral after 5 days. - Propecia/Proscar (Finasteride) You may keep taking your Propecia/Proscar before and after your procedure. - Vitamins You may start re-taking vitamins/supplements 10 days after your procedure. If you are taking Gingko or high doses of Niacin you should wait until 14 days after your procedure before resuming these supplements. - Aspirin You may resume taking aspirin 5 days after your procedure. - Alcohol You can resume drinking alcohol 5-7 days after your procedure. submitted by /u/ArbyKadarby to r/HairTransplants [link] [comments]

When did you know surgery was the right choice?
Hi! I’m a 23-year-old male who might need surgery, and I’ve been questioning whether it’s the right choice. I know this question has been asked many times here, and I’m aware it’s against the rules to ask for medical advice. All I’m hoping for is for people to share their personal stories. Hopefully, this post can be useful not only for me, but also for others in a similar situation. My story is this: I used to go to the gym 6 times a week and did compound movements every single day without proper rest. I had back spasms for months but ignored them because the pain wasn’t too bad once I got warmed up. Then one morning, I deadlifted 120 kg. On the eccentric part of the lift, after getting the weight up, I lost my brace and felt an immediate sharp pain in my lower back. At the time, it didn’t feel too severe. I left the gym and took two weeks off training, but it never got better. Now, 3.5-4 years later (the injury happened on April 18th, 2022), and after visiting 17 different treatment centers, I’m still suffering, and it’s only gotten gradually worse. I was able to travel to America two summers ago, but after trying to be active again and lift weights avoiding any back lifting movements, and only focusing on back supported movements like seal rows and such, it only got worse and I had to cut being active completely excluding small 10-15 minute walks several times a day. I’ve tried physiotherapy, chiropractic care, cupping, acupuncture, ultrasound heatwave therapy, shockwave therapy, stretching, not training, training around the pain, McGill exercises for several months, McKenzie, and more… All without luck if any improvement. They all felt like, they just slowly gradually worsened it. I also had a facet joint injection/blockage consisting of 40ml Triamcinolon and 3ml Marcain, which didn’t work at all. Pain stayed the exact same, which confirms that it’s not the facet joints that’s the issue. Today, I’m lying in bed most of the day - probably around 20 hours - with constant back pain, along with burning and tingling sensations in my legs and feet. My latest MRI from November 14th, 2024 showed mild degeneration at L4-L5, but moderate degeneration (Pfirrmann grade 4) at L5-S1, with a protrusion, and a posterior annular fissure that has been present since my very first MRI in 2022 right after the injury. There is no direct nerve compression, but that doesn’t mean I’m not in brutal pain. Every single day, I wake up pain-free for just a few minutes, and then the cycle through hell begins and lasts the rest of the day. Nights are the worst, where it feels like the symptoms ramp up even more. I’m currently trying to get a referral to a hospital spine specialist, but my doctor refuses because she says it’s “normal” for even young adults to have some degree of degeneration. She just refers to a study that included around 3000 asymptomatic people where up to 30% of 20-year-olds had degeneration. Of course, that can be relevant, but it doesn’t change the crucial pain I’m in. To her, apparently, it’s completely normal to be bedridden all day due to pain - clearly a choice I’ve made myself… I have no social life. All my friends have drifted away. I have no job. Attending physical classes at university has already been taken from me - I’ve been bedridden for about a year now, and even trying to study remotely is becoming impossible as things gradually worsen day by day. It feels like it’s only a matter of time before I can’t do that either, and have to give up on the last thing of my life I still have to some degree. Medication-wise, I’ve tried low-dose naltrexone and currently take 4.5 mg daily with no effect. Baclofen 10 mg combined with 400 mg ibuprofen and 1100 mg paracetamol doesn’t touch the pain either. I’ve just started lamotrigine and am currently at 25 mg, but so far I’ve felt no effect at all. I haven’t tried gabapentin or Lyrica because my pain is primarily centered at the L5-S1 level in my lower back, therefore suspecting it won’t be much help. Also, no matter the medication, it’s only a cover for the real issue. With all the side effects, and the recent study about Gabapentin linked to dementia, I’d rather be in pain till someone takes me seriously than deal with the side effects that would probably lead to me being even more depressed. I strongly suspect the annular fissure to be the main pain generator. Whenever my back pain increases, the leg symptoms worsen as well. I can’t say anything for sure, but it makes logical sense to me. I’ve had ALIF recommended by a few professionals, who I’ve reached out to privately myself because of being proactive about the situation, but nothing has been concluded yet. I’ve also studied ADR abroad as an option to avoid adjacent segment disease, but given how severe things have become, I doubt I could even travel. I live in Denmark where healthcare is free, so if surgery happens here, insurance isn’t an issue. It would only matter if I went abroad. I’ve seen very mixed opinions on ADR. Some people praise it, others say it’s not significantly better than fusion long-term. I hope technology continues to advance, so that if I end up with ASD in 10-15 years, there might be better options available, maybe even injections or other treatments. But I can’t just lie here and wait. I’m severely depressed and completely lacking social interaction. I’ve heard that when pain affects your life to the point where it dictates every single action you take, surgery is at least worth considering. That’s exactly where I am right now. Despite having read all the horrors of fusion surgeries, I’m at that point where it feels like it can’t get much worse. I know it can, but the succesful surgeries are happening, and we just rarely get those stories because they’re out living their life again after being through hell on and on for years. I can’t do anything. I’m 23, and I feel like I’ve lost everything - any hope of a relationship, a career, and all the things most people expect to experience in their twenties. Sorry for the long post. I wanted to describe everything in detail so people could better reflect and maybe share their own experiences or thoughts. Cheers to all the warriors on this subreddit. You are truly inspiring for pushing through every day with spinal pain. submitted by

It's steroid needle day today!
UPDATE: 84 hours post-injection. Sadly I'm calling this steroid shot a big fat failure for me. I'm guzzling down ibuprofen, and if I had opioid painkillers I'd be shovelling them down too. Ibuprofen not holding me at all, it's going to be a rough night, thinking of going to ER for something stronger, then back to my GP when I can get in for Plan B. I have no idea what Plan B even is. UPDATE: 70 hours post-injection. I'm off the narcotic type pain meds but still needing ibuprofen every 6 hours or so. There is some improvement, but this injection is not the magic panacea I hoped it would be. Still in considerable pain at times. I hope things improve, I'm starting PT next week. UPDATE: I'm 48 hours post injection now and still having to take ibuprofen, the pain is not as bad as it has been, but I don't need the opioid painkillers to get on top of the pain. I drove for an hour yesterday with minimal pain, where that had been impossible before. UPDATE: Eight-and-a-half hours in and the honeymoon is over. Back on ibuprofen 600mg at 6pm, 400 at 1am. Suckworthy. UPDATE: PAIN-FREE!! For the first time in six weeks. What a weird feeling procedure though. The local anaesthetic was the worst, I squealed like a teenage girl at a Harry Styles concert. Once that was done, the doctor put in the steroid and I felt a warm, tingling glow go all the way down my L5 nerve pathway down to my foot. Then, zero pain. Apparently it's the local injection that is acting on my nerve to numb the pain and it wears off in six hours, so the pain may return later but the cortisone should start acting in 48-72 hours. For now, six blissful hours ... ahhhhhhhh. I have a broad-based bulge in my L4-L5 disc compressing on the L5 nerve root. It has been six weeks of terrible pain unless I'm doped to the eyeballs. I know all of you know what it feels like. Today I'm having a cortisone shot in my spine. Here's hoping from some relief. I'll report back on how it went. submitted by /u/CorgiCorgiCorgi99 to r/Sciatica [link] [comments]

Some advice from Vector (Dead Spark 6:15-F) for new and old burners.
Hello fellow burners! I am Vector from last year's (2024) camp Sparky's Garage (6:15-F), which is now called Dead-Spark this year, same address. Our camp's mission is to offer technical help for those who found themselves on the wrong side of radical self-reliance. We have fixed just about everything, from broken generators and improperly wired AC units to car lockouts, camp plumbing issues, faulty refrigeration units, and even a broken alternator or two. Sparky's Garage has found a new name this year, "Dead Spark" at the same location, with some added fun, A western theme, a bar, and much more! Come see us for that too! I am writing this note to offer some suggestions of things you should bring to the Playa this year to make our lives a bit easier when things go wrong. This is by no means a complete list, but here are a few things I suggest you do now, not on the Playa: Self: Its going to be hot. Really hot, at times.... BUT it is also can get cold, really cold at night (like in the 40’sF) ! Last year in Reno it was sleeting when I landed. I boarded the burner bus wearing my leather coat and a scarf! Two years ago I was kicking myself for not bringing warmer cloths for the night time. A fellow burner gifted me a fur coat for the evenings, (thank you Josh from Acrobatica Galactica! ) to which I remain eternally grateful. Lesson learned, don't forget to bring clothing for both extremes of temperature. Backpack type water dispenser. This is essential. Dehydration will sneak up on you, and when it does, you are in deep trouble with the closest hospital 2.5 hours away by car. Oh, and one of your crew is sober enough to drive you there right? The medical center on site is very, very good, but please do not give them one more thing to fix, they are busy enough already. Buy the helicopter insurance. ACTUALLY THIS YEAR YOU CANNOT! So, instead, check with your health care / insurance provider and see if you can get a rider for this event. Remember, if you get badly injured (safety third) "they" will put you on a helicopter to the hospital (weather permitting, and availability permitting) If you are in need of this service you will likely not be in a position to accept or decline the HeloEvac. These decisions will be made for you by people who will not be paying the bills. If / when they put you back together again, you will be facing a bill in the many tens of thousands of dollars just for the ride out. Do yourself a favor and buy one less outfit, and instead contact your insurance provider and ask if you can buy extra coverage for this week. Prescriptions. make sure you are not going to run out of your medications while on playa. Also, check your medications and see if they have incompatibilities / complications with your bodies exposure to extremes of sunlight, heat cold, and dryness. Common medications that are especially challenging in the desert include 1) Insulin. Must be refrigerated. What’s your plan here? Is your generator / cooler up to snuff? 2) Antibiotics: Tetracyclines, Doxycycline, minocycline. All of which are very much affected by sun exposure. I AM NOT A DOCTOR, (but I do play doctor….) so if you are bringing prescriptions to the desert have a quick chat with your doctor first. # Car: Hide-a-Key: Use a hide-a-key and put a spare somewhere you can find it when you lock yourself out. Is your spare a chip key? Can it actually start your car or just get you in it? Jumper Cables (better yet Jump-Pack) and Mini Air Compressor: Bring these if you don't already have them. Fluids Check: Check the oil and other fluids before you drive in. Fill up before you get in, but not at the last possible stop because the last gas station is a shit-show of the unprepared. Space Blankets: Buy a couple of space blankets and some strong magnets to cover your windshields. Your vehicle may become your home if something goes terribly wrong with your tent. Use plenty of magnets; you don’t want that space blanket blowing off and becoming trash. Tires: How are your tires? Is your spare tire full of air? Can you get your spare tire off of its mount, or has it rusted itself in place? Take a very good look at your tires, are they dry rotted? Are there little cuts in the side-walls? Are any of them starting to de-laminate? Does your car shake at highway speed? All of these are indications of tire issues. I learned this the hard way in a 400 mile trip to an east-coast burn. Don’t be me. Just because the tread pattern is good (as it was on all 6 of my tires) does not indicate that the tire is healthy. I had de-lamination issues that I was un aware of from tire age, and suffered a minor blow out on the way to Transformus-24. Shame on me for not spending more time looking at the tires and denying that the slight shake was more than just a "balancing issue" Tow Strap/Chain: We all remember the mud of 23... Obvious. Car A/C: Does your car A/C blow really cold? Not a great idea to run your A/C in the desert while stopped in line during a dust storm, but if it is not blowing cold, that may be a sign of trouble for your cooling system (bad fan clutch/relay/fuse, etc.). No better way to find cooling system trouble than in a 3-mile-long line in the desert while trying to get into Burning Man. # Generator: Mouse Nests: Check for signs that mice have built a nest in the top of the motor. Look for twigs, lint, or anything else a mouse might use to make a comfy home. They seem to love doing this, creating a situation that causes your engine to overheat and destroy itself after about 15 minutes of "flawless" run time. If you get to this point We (Dead Spark) will not be able to help. The motor will be toast. Now, at home, take a can of compressed air and blow off the fins on the motor, look for mouse nest parts, and use a straw/small hook to make sure you get all of the pieces of that nest out of the motor if they are present. If you don’t your burn will be way more difficult than it needed to be. Air Filter: If your generator has a foam air filter (most small inverter generators do), open it up and look at the filter. Has it degraded to dust? Is it mushy and not foam-like? These filters tend to degrade over time and turn to loose fluff. Once they degrade, and you start the generator, you will instantly suck that fluffy dust foam filter right into the carburetor and create a dead-generator situation. Verify your filter is not dust, wash it (if foam) or blow it clean (if paper), take a picture, and buy a spare that matches your picture. Verify that you got the right one as the 17 year old at the engine parts counter doesn’t give a shit if you find yourself SOL with the wrong filter on the Playa, and Amazon wont deliver. Spark Plug: Look at your spark plug. Get the part number off of it and buy THREE spares. Bring two them to the Playa. (you are going to use the third one in step 4 below) If you want to be super nice, bring one of your spares to Dead Spark on the Playa (6:15-F) and tell them that they can use it for someone in need. If you want to be extra-nice bring us a can of carb-spray. We never have enough. Spark Plug Replacement: Change the spark plug on your generator (YouTube it). Keep the old one and the extra two new ones. Tape one of them to the side of the generator so you don’t forget them at home, and so you can instantly find them when you are in need. Oil Change: Change the oil on your generator. If you don’t know how to do this, go on YouTube and learn how. This is a valuable life lesson. Bring 2 extra quarts of oil. Hopefully, you will not need it. Carburetor Cleaner: Buy a can of carburetor cleaner. (Actually buy two, and come and gift us one.. We always need more carb cleaner!) If your old-ass gas has gummed up your generator, the person fixing your carburetor (possibly me) will very much appreciate that you brought extra carb spray. You can also sniff it and get high. (Do NOT sniff carb spray to get high! Rookie move. Experienced burners know to sniff the starting fluid from step 7 to get a proper high.) Starting Fluid: Bring a can of starting fluid. DO NOT USE IT UNLESS YOU KNOW EXACTLY WHAT THE F\*\*\*K YOU ARE DOING. This is for the person fixing your dead generator. Starting fluid is very dangerous in unskilled hands. (and for real, do not sniff this explosive crap either!) If you have an RV with a built-in generator, like an Onan, consider buying an extra fuel pump and an extra fuel filter. Onan generators use a proprietary fuel filter that screws into their proprietary fuel pump. If you’ve never replaced this filter, do it now, while you're at home with good tools. Keep the old parts and bring them as spares. I see at least 10 of these pumps fail on playa every year—and that’s just the ones I personally encountered. I’m sure there were more, since I didn’t meet all 75,000 people at Burning Man. If you don’t want to pay for the official Onan pump, go on Amazon and buy a LOW PRESSURE electric fuel pump as a "half-ass, at-least-get-it-to-work-on-playa" backup. If you're too cheap to even do that, at least bring 10 feet of fuel hose and a gas can. That way, as plan C, you can run the generator using a gravity-fed siphon setup if needed. New advice for 2025: If your RV generator has an easily accessible fuel line, (they usually do) consider splicing in a generic inline fuel filter as a pre-filter, ahead of the weird, hard-to-replace Onan filter. Bring a spare. Let this easy-to-access, easy-to-change filter be the one you actually need to service on playa. Test Run: Go start your generator. Plug in an electric heater (make sure this heater is a t a safe distance from anything flammable!) or a similar constant heavy load. Let it run for an hour. How’s it doing? Running smoothly or giving you cause for concern? If it’s running poorly at home, it’s not going to work any better on the Playa. Equipment Testing: Plug into your generator the things you plan to run on the Playa. For example, the fancy inverter-window-air-conditioner and the Gucci electric ice maker you can’t wait to show off to your campmates. Do these items mysteriously cause the GFI breaker to trip on your new Harbor Freight generator? Best to find this out now. TEST your inverter style AC by temporarily plugging it in to a GFCI outlet at home and letting it run for at least 10 minutes. Then do this with your generator. (Most inverter setups do not start the compressor for the first 5 minutes after power is applied) Is the compressor running? Is cold / hot air coming out of it the way it should? See how /if it works. Remember burning man is at high altitude. We saw more than a few 4KW generators that "worked fine at home" become too weak to run the RV air conditioner in the high desert. Test, adjust and Make a plan. # Air Conditioners/Large Refrigeration Units for Food Service Camps: Animal Nests: If your RV has an old-school rooftop unit, open it up and look for animal nests. Birds and squirrels love these things for some stupid reason. Capacitors: Look at the starting/run capacitors. Are they 15 years old, having lived in direct sunlight with no relief? You may want to replace them now while you are at home and can get parts easily. Bring the old ones to the Playa. Someone else may need them. A used capacitor has great gift value on the playa for someone who has a bad capacitor. Fins Inspection: Inspect the fins inside and out. Are they full of animal hair and dust? Fix that. Never cleaned your filter before? Might want to find and clean it. Start Relay: For large industrial fridges/food service-type refrigeration, how old is your start relay? I saw one of these go bad with Playa dust in it last year. If you can get a spare, bring it. As a plan C, we taught the owner how to become his own start-relay with a suicide-cord setup. No one got electrocuted to the best of my knowledge, but it was far from ideal. Inverter-Style A/C: Again, if you bought a fancy new inverter-style air conditioner, be aware they tend to trip GFCI breakers/outlets. Test your A/C unit with the generator you are using. If your generator only has GFCI-protected outlets, and your A/C is an inverter-style A/C, you may see the GFCI trip. If you plan on plugging that inverter-style generator into your camp’s big-ass generator, make sure you are not going to trip the GFCI feeding your section of the camp. If, in deed you are having GFCI issues you may need an isolation transformer to solve the issue. (DM me if that’s the case, but fond out NOW, not on playa) # Plumbing: Plumbing Test: Test your camp/RV’s plumbing now, not on the Playa. Did your piped freeze and crack because some family member emptied the cooler in the sink after you winterized it? Water Filter: Check/clean your water filter. Is it a cartridge type? How many spares do you have? Spare Water Pump: If you can, bring a spare RV/camp water pump. The diaphragms in the pumps tend to harden with age and only reveal failures after being used a bit, usually when you are on the Playa. Plumbing Supplies: Find out what kind of plumbing you have (Pex, PVC, etc.) and bring some extra fittings and pipe. This stuff is cheap at the big-box store and priceless when needed on the Playa. # Electrical/RV: Battery Check: How is the battery in your RV? When was the last time it was replaced? Do you have jumper cables? Fuses: What kind of fuses does your RV use? Get a box of spares. Bus Bars: Check your bus bars (if you have them). Are all the connections still tight, or did they vibrate loose from your last trip on Rt 49 Jungo Road? Battery Charging: Do you remember how to charge your engine battery from your house battery? Does your generator still charge the house battery, or is the charger not working properly? Check your battery chargers; the capacitors in them can go bad. This is usually found out on day 2 of the playa. First Aid Kit: How old is your first-aid kit? Are the band-aids still sticky? Is the bottle of Bactine from 1997 still good? What, exactly is the shelf life of ibuprofen capsules? Tail Lights: Do all of your tail lights work, or are you giving the cops a reason to search your RV? Spare Tire: Is there air pressure in your RV’s spare tire? Can you get it loose from the tire mount, or is it rusted solid? Does your RV have dually tires? Can you actually fill your tires with the Wal-Mart air pump you bought? Dually tires are a bitch to fill up when you have the proper tools, are you equipped to fill them on playa? Bottle Jack: Does your RV’s bottle jack still work, or is it simply a tool for spilling oil on the side of the highway? # Bikes: There are many bike repair camps on the Playa that can speak to this better than I can, but I do want to mention the following: E-Bikes: Just don’t. I know you want to, but just don’t. Almost none of the bike shops on playa will service these, and everyone you pass secretly hates you—not because of you, but because of what that other idiot E-biker did to them last year. Burning man is not a sprint, it’s a marathon. Spare Battery: If you’re not listening, bring a spare e-bike battery if you can. Generally, there is no fixing the battery packs. They are usually potted or sealed shut. Even if you get them open, are you going to spot weld in a new 18650 cell or fix the BMS? (Digikey does not deliver to the Playa) Charger Connector: How does the connector look on your charger? Does it look like my 14-year-old's phone charger, all kinked and frayed? This may be a sign that you should replace it NOW, as Amazon does poorly on Playa delivery as well. Front-Wheel Motor: If your e-bike has a front-wheel motor, your front forks are going to take a beating on the Playa. The front forks of bikes were never meant to have this much pulling force. I have seen more than a few broken front forks from the motor forces versus the playa. The pulling force and rough surface of the Playa causes them to fail. Go easy on this setup if this is what you have. Manual: Bring your e-bike’s manual. It may be helpful with error codes, etc. As an added bonus the broken English is fun to read. Backup Plan: When your e-bike fails you, you will be slowly pedaling a bike that is way heavier than the dirt-simple two-wheeler your campmate is riding to the trash fence to see the last 10 minutes of Daft Punk. You are going to miss the show, all because you brought an e-bike. Daft Punk was incredible last year, don’t miss it because you insisted on bringing an E-Bike. I hope these tips help you prepare for the Playa and make your experience smoother and more enjoyable. Stay safe and have a fantastic time at Burning Man! My Comprehensive Guide To Eczema
Background: I am currently at WAR with my 3 year old daughter’s eczema. I have been severely depressed watching her suffer but I am now channeling my sadness to anger and determined to knock this bitch into remission. Not to be whatever but people have told me I’m the most obsessive person they know, which usually doesn’t serve me well. However, in this particular instance it’s actually quite helpful. So here is my comprehensive guide (after obsessively reading countless websites, articles, studies, and almost every single post on Reddit) to treating eczema that is balanced between holistic and medical approaches. I feel like everything I’ve read leans heavily one way or the other so wanted to list out everything possible from both sides of the spectrum. I definitely do give a lot of opinions, they are not facts, I am open to being challenged if you think I’m incorrect about something. I also really wanted to build something comprehensive for all of the teens on here suffering mentally from this awful and life-altering illness. If you are one of those teens that are reading this right now, please know there is hope. There is always another treatment, another lifestyle change to try. Please don’t give up. PM me if you need a supportive adult. I gotchu. Where to start: Doctors: Dermatologist. This may seem obvious but dont rely on your GP for eczema. I recommend if you are in the US to find a provider through the National Eczema Association. Namely because these doctors acknowledge TSW and are also always evaluating the latest treatment options. If you are currently being seen by a dermatologist that doesn’t recognize TSW, find someone else. Seriously. Im not anti-steroid, but I didn’t know what TSW was until recently and I’m horrified how nonchalantly I used steroids on my daughter. Allergist. Ugh we’ve had to advocate for ourselves a lot here. The 2 allergists we’ve seen have been really adamant about not testing since they didn’t want us to cut out foods completely from my daughter’s diet. But keep pushing them. Tell them you’ll go 98% allergen free or whatever and continue eating foods you are sensitive to. Don’t stop pushing them until they test you. They are going to love to tell you that food doesn’t cause eczema. I mean sure, that’s technically correct, but it can be a huge trigger for a lot of people. I don’t care what they say. I’ve combed through so many comments on here where people have reduced their eczema significantly through removing foods they are sensitive/allergic to. Also, anecdotally it’s worked for every single person in my family who’s had eczema. Vitamins/supplements: D3: Everyone, and I mean everyone, who suffers from eczema should be taking D3. Babies, 400 IU per day. Children, 1000 IU per day. Adults, 4000 IU per day. You can get a prescription for this if needed. Several people on Reddit have seen major improvements by simply taking D3. Also for the parents reading this and thinking to themselves that their child gets enough vitamin d from milk, nope. 8oz of milk is only 100 IUs of vitamin D. Fish oil/Omega 3: I can’t get my kids to take this lol but if you are an adult, you should def take fish oil supplements. Zinc: everyone who suffers from eczema should be taking zinc. Probiotics: everyone who suffers from eczema should be taking probiotics. Try and research probiotics that are good for skin and combating staph. Melatonin: this helps get you to sleep but also helps with nighttime itching. Won’t change your life but will help. note: too much vitamin d and zinc may cause gastrointestinal issues! Creams/lotions: Vaseline. If you or your child are in a flare, this will probably be the only thing that won’t burn or sting. It now comes in a stick form which is great for travel. Vanicream Cerave healing ointment (dark blue tub) Avene cicalfate. I know, I know. This stuff is pricey. But it’s the only thing that doesn’t hurt my daughter when she’s having a flare. Everything else. It’s worth trying it all. I know it gets expensive but once you fine the right cream, it can make a world of difference. If you live in the US you can return everything that didn’t work for you. Don’t feel guilty. amerigel - this stuff is used on really bad wounds, like diabetic neuropathy stuff. It creates a wet environment beneath it like a liquid bandaid. Used sparingly on bad patches of eczema where it is extremely dry, it seems to really help. note: colloidal oatmeal is in sooo many of the creams/oitnments/soaps marketed directly for eczema. BUT some people have a sensitivity to it and it can cause a flare. My daughter is one of them and we figured that out the hard way! Treatments: Prescriptions: Topical steroids: obviously this is the first line of defense for eczema and usually the first thing your doctor will suggest. And look I totally understand it’s a sensitive subject for a lot of people on here. But there is a place for steroids for some people. If you are a parent of child with eczema reading this, please don’t go above a medium potency steroid. I’m not a doctor, so this is not medical advice. But after reading thousands and thousands of comments on Reddit, this is advice I feel comfortable sharing. Also, always talk at length with your doctor about how to properly utilize topical steroids and follow that religiously. I really think topical steroid prescriptions should be accompanied with a journal to track usage, life gets busy and it’s a good practice to journal while you use topical steroids to avoid misuse. Biologics: ask your doctor about dupixent, adbry, ebyglyss. These are monthly shots which I know can be intimidating but they are miracle treatments for a lot of people. Also fully recognize prescriptions for these outside of the US are difficult to obtain. However, bright spot is that from what I see on Reddit, biologics are being utilized more and more around the world. Another bright spot. There are currently 39 biologics in development according to the national eczema association!!!!! A lot of them are already ready for prescriptions! So this means if dupixent didn’t work for you there are lots more on their way. Topical Jak inhibitor: ask your doctor about Opzelura. Topical calcineurins: ask your doctor about Tacrolimus and pimecrolimus. Topical PDE4 inhibitors: ask your doctor about Eucrisa and Zoryve. Immunosuppressants: cyclosporine and methotrexate are utilized when you need to get a flare under control. Jak inhibitors: ask your doctor about Cibinqo and Rinvoq. Oral steroids: ah I know this may get me some heat in the comments but if you have not gone down this path, I would strongly urge you to consider anything else. Literally anything else. I personally would never use for my child. Phototherapy: I know this a miracle treatment for lots of people. But if you have a child, I would strongly urge you to not go down this treatment path until they are older. PM if you want to hear my experience with my daughter. This brought me a great deal of relief during dark times. Treatments in development: https://nationaleczema.org/research/eczema-treatment-research/ OTC: antihistamines: can definitely help with itch and reduce inflammation. Ibuprofen: for pain during a flare Other treatments: wet wrap therapy: if you have not done this yet, please do. Just don’t use topical steroids when doing wet wrap therapy. I’ve ready they can increase potency by 600x when used in combination of wet wraps. Bleach baths: everyone with eczema should try this, it helps get rid of staph. Consult with your doctor on how to safely do a bleach bath and whether or not your skin is too sensitive for this option. Apple cider vinegar baths Dead Sea salt baths lots of people in the UK recommend light paraffin baths. Brand I see the most is Oilatum. Hypochlorous acid: can be bought on Amazon. Same concept as a bleach bath but more gentle on the skin. Oatmeal baths (be careful if you have a gluten intolerance/allergy) Cold compresses/ice packs to help control the itch black tea compresses cryotherapy red light therapy Ozune iv therapy Eastern Medicine: TCM: can be very expensive but also a life long cure for people. The protocol is often times intense requiring multiple baths a day etc. But again, could potentially bring you life long relief. There are only two doctors that I have seen constantly been praised by people online- Dr. Xiu Min Li- she is located in NYC, but does take zoom appointments and treats patients from all over the world. You can join the Facebook group “TCM for allergy and immunology” to learn more and how to book an appointment (wait time is very long). From what I’ve seen she’s a miracle worker. Dr. Olivia Hsau Friedman- she is located in Chicago. https://amethystacu.com/about-2/ Acupuncture: technically part of TCM. if you find the right person, it can bring you a great deal of relief. Ayurveda: have not read much about this, interested in hearing what people’s experience with Ayurveda has been. Treating it like a Staph Infection: Read this Reddit post: https://www.reddit.com/r/eczema/s/wdZSoGHH5J From what I’ve gathered this is how you start this method: Body wash Wash with a chlorhexidine gluconate (Hibiclens) or triclosan body wash daily for five days. Start at the neck and work your way down, paying special attention to skin folds. Avoid getting the soap in your eyes, ears, or mouth. Nasal ointment Apply a thin layer of mupirocin (Bactroban) ointment to the inside of each nostril twice a day for five days. Use a cotton swab to apply the ointment and pinch your nostrils together to massage it in. Sheets and pillowcases Wash or change sheets and pillowcases daily while using the ointment and body wash. Topical probiotics: I ordered the skinsea defensin. Would love to know if anyone’s tried any others that have worked for them! Triggers: Food Allergens: Get an allergy test. Eliminate foods that you are sensitive to/allergic to. Keep at it for a while (2 months minimum). Carnivore diet. Try at this for minimum 3 weeks. It seems to reset people’s gut and has worked for people on Reddit. Worth a try if you are at your wits end. Nightshade vegetables. Try not eating nightshade vegetables. It randomly works for people and easy enough to try. Gluten. Gluten seems to be a big trigger for a lot of folks. Try cutting it out for a month. See if it helps. Dairy. Dairy seems to be a big trigger for a lot of people. Try cutting it out for a month. See if it helps. Eggs. Eggs seems to be a big trigger for a lot of people. Try cutting it out for a month. See if it helps. Nuts. Nuts are a big trigger for a lot of people. Try cutting it out for a month. See if it helps. Environmental Allergens: get an allergy test for pet dander, pollen, dust mites and mold. Pet dander. If this is your allergen, good news, easy trigger to avoid. Bad news, may be a very emotional trigger for you to avoid. Pollen. Taking Zyrtec seems to really help people if this is your trigger. Also consider immunotherapy. Mold. Mold is one of the most annoying triggers imo because it’s expensive to figure out if you have it in your home and also you may need to move which is not a reality for some people. You can get a blood test for mycotoxins. If you are someone who’s flares go away on vacation or when you stay at a friends house, mycotoxin test may be worth looking into. I also think if you are allergic to seasonal mold, Zyrtec and immunotherapy are options for you. Dust mites. Without a doubt the biggest bitch of a trigger to contend with. I have read that comment from the guy who literally moved from Singapore to Michigan several times, he seems much happier though FWIW. If you can’t migrate. Get an air purifier with hepa filter, vacuum with hepa filter, don’t make your bed, get dust mite covers for your mattress and pillows, get rid of curtains, get a dehumidifier and wash all your bedding with hot water and as often as you can but min 1x a week. Immunotherapy and Zyrtec are also an option for this. Irritants: Laundry detergent: this is a big one. I think the only truly safe brand after reading hundreds of comments is Molly’s suds. Dryer sheets: no safe dryer sheets, if you have eczema do not use these. Fragrances: best to avoid completely. Everything you utilize should be fragrance free. YOU GOT THIS. submitted by /u/No_Educator1574 to r/eczema [link] [comments]

Scz fra vina mea că am ciclu. submitted by /u/aggressive_cloud to r/WomenRO [link] [comments]

3 weeks in Europe with a 23L Rolltop as a T1D | Quechua Escape NH500 Rolltop
🎒 23L Backpack │ ✈️ Netherlands, Ireland, England │ 📅 Mid-June to Early-July Hello fellow OneBaggers, I understand I'm only 6 months late, but here's my post-trip report of my OneBag experience during a quick 3-week-long trip to the Netherlands, Ireland and England in June-July 2022. The weather varied greatly but fortunately I'm naturally always super hot so I was quite comfortable in Ireland and England! I am a T1D cyborg so there are some critical medical supplies I need to carry with me, on top of having an electronic device embedded in my arm (that I have to replace every 14 days by embedding a new one in the other arm) called a Continuous Glucose Monitor that allows me to have a super easy and quick reading of my blood sugar and see the trends (going up, going down, stable). I need to carry insulin and its supplies with me at all times, and also carry some emergency quick acting carbs incase my blood glucose is going dangerously low - while I usually see the trends before being in hypoglycemia, I have the opportunity to buy a drink or simple sugar snacks in an urban setting - but when I'm in the middle of nowhere where no shops are in sight, I'll need to have emergency carbs. While I usually always have a backpack on myself - I tried to do things differently for this trip - I had my backpack, but I mostly left it at my accommodations - instead I carried a waist pouch with me at all times - much smaller, freed my back (I GET HOT REALLY EASY.) and felt lighter too! Without further ado, here's my complete loadout - happy reading! ⚠️ For an extremely abridged and to-the-point packing list thread, click here. ⏱ 25 min reading time │ 🖼️ 16 Pictures │ LOADOUT WORN OR CARRIED ON ME - During the departure day 🖼️ Spread out | ⚖️TW: 400 g The nylon belt was perfect for airplane travel as it contains no metal part - I never had to remove the belt in any of the airport security checkpoints. The Henley was light enough to not get TOO hot by simply rolling the sleeves. The convertible pants added great versatility by converting to and from long to short pants - plus, having the ability to unzip around the knee, also allowed me to take my insulin in the thighs without having to remove my pants. Trail running shoes are so perfectly versatile, breathable and comfortable, they're quickly becoming staple in my travel loadout - whether walking in the city or in the jungle, they're ideal. So from head to toe, here's what I wore or carried 1 × Reusable Face Mask 1 × Freestyle Libre Continuous Glucose Monitor - (embedded in my arm - I'm a cyborg) 1 × Amazon Essentials Waffle Henley - Jade Green 1 × DECATHLON Hiking Merino T-Shirt - Blue 1 × MEC Mochilero Stretch Convertible Pants - Tan 1 × Nylon non-metal Belt - Navy 1 × UNIQLO Supima Boxer Briefs - Stripes 1 × Sports Ankle Socks - Black 1 × DECATHLON XT7 Trail running shoes POCKETS 1 × Passport 1 × Cellphone BACKPACK - Decathlon's QUECHUA NH500 Escape Rolltop 23L - Yellow Ochre 🖼️ Unpacked | ⚖️TW: 7 kg / 15.5 lbs While I usually like being discreet, I decided to go with a high visibility and easily recognizable colour, as I had to take the plane multiple times and expected to use the over-head bin. Having a high-viz bag would make it easier for me to spot if someone tried to steal it. Furthermore, I added a tiny Gear Aid Ni-Glo Gear Marker on the front of the backpack for customization. This little accessory glows in the dark for hours too! I was very vocal against the Scrubba Washbag due to the pricing, but I got gifted one months before my trip - other than the price, it is a solid accessory for travel and I used it almost every night Upon departure, the bag was loaded with: 1 × Tech Pouch 1 × Toiletry Kit 1 × Medium Medikit 3 × Medium Packing Cubes 1 × Nintendo Switch Carry Case 1 × Travel Passport ''Wallet'' 1 × Sleep Kit aka The Sandman's Pouch 1 × LABO MONO Packable Rain Jacket 1 × ZENDURE Passport II Pro Universal 3-in-1 Power Converter, Adapter and USB Hub 1 × Scrubba Wash bag WAIST POUCH - A discontinued/old version of the Pacsafe METROSAFE LS100 GII 🖼️ Unpacked | ⚖️TW: 1 kg / 2.2 lbs The version I own has long been discontinued and has a belt pass-through and a different design [the current version doesn't have a belt pass-through anymore]. I cleverly made use of the belt passthrough combined with the included removable shoulder strap to use it as a waist pouch - the belt I was wearing wasn't used with the pouch, the shoulder strap inserted through the belt pass-through was wrapped and locked around my waist, so I could carry it without having to wrap it through my pants' belt loops - it was simply resting around my waist. Quick to remove for airport security checks too. It was slim enough that I could comfortably wear it whilst seated in my airplane seat. In terms of theftproofness and safety, the zippers can be secured with a hidden lockable hook, the bag is made of exomesh within the fabric's layers and the strap is slash proof - it is a PACSAFE product after all - so I was never worried about pickpockets, whether the pouch was located on my side or my back. It contained: 1 × Insulin Pen Case 1 × Small Medikit 1 × MiniX2.0 Slim Wallet 1 × ANKER Soundcore Life P3 Earbuds 1 × Mini Notebook & Pen combo 8 × Hand wipes (4) & alcohol wipes (4) 4 × GU Campfire S'mores Energy Gel Pouches 1 × Folding polarized Sunglasses 1 × 19L Ultra-light, ultra-foldable Nanobag 5.0 - Dark Navy │ DETAILED BREAKDOWN TECH POUCH - Decathlon FORCLAZ Travel Rigid Case 🖼️ Unpacked | ⚖️TW: 330 g I actually gave this case to my mother that I met in the Netherlands during the first few days of my stay as she was going back to Canada and I didn't need any of its content after all - it was extra weight and used up space for nothing. Well, it did have little use - I helped my mother back up her thousands of pictures on the hard-drive so she'd have storage space on her phone ahahaha 3.5mm Audio Jack earphones w/ microphone USB-C earphones w/ microphone 128GB USB-C & USB-A dual thumb drive 1TB USB-C Solid State Drive USB-A to USB-C cable USB-C to micro-USB adapter Micro-USB to USB-C adapter USB-C to USB-C cable (short) USB-C to USB-C cable (extra long & extra durable) USB-C to 3.5mm audio jack adapter TOILETRY KIT - Mytagalongs Network Case - Unplug Silicone 🖼️ Unpacked | ⚖️TW: 620 g I own an old version with no handle at the top and the bottom compartment has a separator to create 2 pockets. I had access to showers, soap, shampoo at all the accommodations I was, but this toiletry kit always travels with me. I did use the Camping Soap for handwashing my clothes. This kit is also always in my backpack, whether at work or going at a friend's - the only thing I add to it while traveling is a Shaver or a Trimmer - for this trip, I went with a trimmer. Charged in June for the trip, the battery is still running on that charge, in December! Toothpaste Deodorant - Old Spice Captain (I have strong adverse reactions to antiperspirants) Electric Toothbrush - Philips One Biodegradable Camping Soap Lip balm Nail Clipper Reusable Earswab - LastSwab Philips Multigroom Series 5000 Trimmer 4 × Alcohol swabs 4 × Dental Floss Picks 10 × Acetaminophen Tabs in a tube 12 × Pen Needles in a kit MEDIUM MEDIKIT - MEC First Aid Bag - Small 🖼️ Unpacked | ⚖️TW: 415 g The joys of being T1D. Need to carry a crapton of medical supplies to stay alive. My CGM lasts 14 days, and the one I had would expire the next Sunday following my arrival, only brought 1 replacement to cover the full 21 days of travel. Didn't bring a spare - my redundancy would simply be to use the conventional finger-pricking blood testing method if I had issues with the CGMs 100 × Injection Needles (I use 4 a day [84 for 3 weeks] and brought redundancies) 20 × Self-adhesive bandages of various sizes 20 × Alcohol swabs 10 × Cleaning wipes 15 × Acetaminophen Tabs - Tylenol 15 × Acetylsalicylic Acid Tabs - Aspirin 15 × Ibuprofen Tabs - Advil 3 × Bolus Insulin Vials - NovoRapid 2 × Basal Insulin Vials - Basaglar 1 × Continuous Glucose Monitor Sensor Kit - FreeStyle Libre Prescription cards MEDIUM PACKING CUBE A - Knack Bags Medium Packing Cube 🖼️ Unpacked | ⚖️TW: 1000 g The Knack Bags Medium Packing Cubes coincidentally perfectly fit inside the QUECHUA Escape NH500 Roll Top backpack - exact width and length - so they could stack really well on top of each other and helped maintain the backpack's shape. The undershirts were usually worn as regular shirts 5 × Sports Ankle socks - Black, Blue, White 5 × Boxer briefs - Green, Red, Blue, Navy, Dotted 2 × Airism Undershirts - Black, Navy 1 × Performance T-shirt - Cobalt Blue MEDIUM PACKING CUBE B - Knack Bags Medium Packing Cube 🖼️ Unpacked | ⚖️TW: 990 g The tank top and shorts were used as pyjamas when I slept at my friend's, and at the dorm hotel 1 × Long-sleeved linen button shirt - White 1 × Linen pants - Tan 1 × Tank top - Black 1 × Sports shorts - Black 1 × Cotton60%/Polyester40% T-shirt - Teal MEDIUM PACKING CUBE C - Knack Bags Medium Packing Cube | ⚖️TW: 1200 g I was actually acting as ''courier'' for a friend - she had moved to Ireland but had little-to-no summer clothing and was expecting to visit Italy later that summer - as I would visit her, I planned on bringing her her much needed Summer Clothes and Dresses. This 3rd packing cube was SATURATED with a bunch of clothing and used 1/4th of my backpack space, approximately. Ireland was my 2nd destination after the Netherlands so I had this 3rd packing cube that I never used for 2 weeks. I am honestly impressed by how much clothing I could fit and compress in that packing cube - it contained well over a dozen items. 15+ × Women summer clothing items and dresses that probably wouldn't fit me, bummer. NINTENDO SWITCH CARRY CASE - Nintendo Branded Switch OLED Travel Case 🖼️ Unpacked | ⚖️TW: 570 g I didn't use my NINTENDO SWITCH at all ahahaha - I thought I'd use it during my various airplane transits, but nope. Still, I kept it with me for the entire trip - takes very little space as it is flat. It is the 2nd or 3rd trip where I bring a SWITCH and end up not using it - perhaps one day I'll learn... 1 × Nintendo Switch OLED w/ Joy-cons attached 3 × Games (I had 3 physical cartridges and numerous digital games) TRAVEL PASSPORT WALLET - A discontinued MEC Travel Passport Wallet 🖼️ Unpacked | ⚖️TW: 290 g This neat little passport/document wallet allowed me to keep all the important documents, passport photocopy, tickets, bills, etc. and all in one place. I'd usually store my Passport in it as well, unless I had to use my passport within the next hour - in those situations, the wallet would be in my pants' zippered pocket for quick access. 1 × Passport 1 × Travel WiFi Sim Card Kit 1 × Pencil & Blank papers Some cash money Various boarding passes, tickets Vaccine documents & other documents Prescription cards SLEEP KIT AKA The Sandman's Pouch 🖼️ Unpacked | ⚖️TW: 90 g This pouch contains items dedicated to help me fall asleep and adjust to the changing time zone and reduce jetlag. I only consume a single melatonin tab when changing time zones. Melatonin pills are NOT designed as medium or long term sleeping aids. They are designed to adjust to a changing sleeping schedule, they are not designed for multi-day consumption. Herbal tea contains no caffeine and kinda helps in relaxing, and the eye mask and the earplugs are to reduce or outright block sensory stimuli 10 × Melatonin tabs (3 mg) 5 × Herbal tea bags 1 × Eye mask 1 × Reusable earplugs set INSULIN PEN CASE 🖼️ Unpacked | ⚖️TW: 170 g I require insulin injections to survive. Without insulin, I'll die a slow, agonizing death through Diabetic Ketoacidosis or its numerous complications. My daily routine includes filling the case with 4 new needles, as I require 3-4 a day. 1 × Rapid/Bolus Insulin Pen 1 × Slow/Basal Insulin Pen 1 × Emergency Carbs (at least 10g) 2 × Alcohol Swabs 4 × Pen Needles SMALL MEDIKIT 🖼️ Unpacked | ⚖️TW: 140 g This kit contains my CGM [continuous glucose monitor] contingency - if it fails, I simply manually check my blood glucose with a conventional glucose meter kit. Also includes some extra supplies. 1 × Blood Glucose Meter 1 × Lancing Device 5 × Lancets 50 × Glucose Testing Strips 4 × Alcohol Swabs 4 × Pen Needles SLIM WALLET 🖼️ Unpacked | ⚖️TW: 50 g I do not really change its content even when travelling - this is what it contains 99% of the time whether at home or abroad. I usually use my Cellphone for Google Pay-compatible payments, and rarely-to-never carry cash money in my wallet. ID with picture (Health Insurance Card) Bus Card Credit Card Debit Card House Key SIM Ejector Tool BACKPACK LAYOUT LAYERED FROM TOP TO BOTTOM OF THE BACKPACK 🖼️ Packed | 👓 ''X-Ray'' The backpack offers a full length front zipper that allows access to the backpack's main compartment without opening the rolltop. Furthermore, you can also access the main compartment through the laptop zipper, but you have to go over the laptop pocket, so can only retrieve smaller items from there. Also, perfect coincidence - the packing cubes have the EXACT dimensions of the inside of the backpack, so they fit perfectly! They'd stack wonderfully on top of each other occupying the full width and length L6 - Medium Medikit, Packable Rain Jacket L5 - Toiletry Kit L4 - Tech Pouch, Sleep Kit, 3-in-1 Adapter, Nintendo Switch Case L3 - Packing Cube A L2 - Packing Cube B L1 - Packing Cube C Front Pocket - Passport Wallet (you can stow/hide the zipper pull tab within the pocket so it becomes theft resistant - the front pocket pretty much becomes invisible as there are no visible seams or zipper lines) Side Pocket - Scrubba Wash Bag │ WEIGHT BACKPACK & CONTENT Weight (g) Weight (lbs) Backpack 816 g 1.79 lbs Tech Pouch 330 g 0.72 lbs Toiletry Kit 620 g 1.36 lbs Medium Medikit 415 g 0.91 lbs Medium Cube A 1000 g 2.20 lbs Medium Cube B 990 g 2.18 lbs Medium Cube C 1200 g 2.64 lbs Nintendo Switch Carry Case 570 g 1.25 lbs Travel Passport Wallet 290 g 0.63 lbs Sleep Kit 90 g 0.19 lbs Packable Rain Jacket 540 g 1.19 lbs 3-in-1 Universal Power Converter 170 g 0.37 lbs Scrubba Wash Bag 65 g 0.14 lbs BACKPACK TOTAL START 7.0 kg (7096 g) 15.57 lbs BACKPACK TOTAL END 5.3 kg (5346 g) 11.68 lbs ⚠️ Few days after my arrival, -300 g from the tech pouch given to my mother - I kept a USB-C to USB-C cable. I made sure the Zoos I visited had adequate accreditations and offered proper ethical treatment of the animals), Three Museums, Various Markets, Countless Restaurants, Numerous Sites (and Sights) and an average of 23 000 steps a day, I've ventured solo (for 90% of the trip) in the Netherlands, Ireland and England from Mid-June to Early-July. I've stayed in the Netherlands for about 2 weeks, and spent a few days in Ireland and England. While already familiar with the Netherlands, it was my first time in both Ireland and England, and I definitely have to go back for more than a few days... ✈️ TRANSIT & TRANSPORT I bought my main round-trip ticket from Canada to Netherlands to Canada some weeks before the trip itself, and while I knew I wanted to go to Ireland (visit a friend) and England (visit Lara Croft), those plans were not definitive - or rather, the dates were not definitive, so I bought those tickets when I was in Europe. As it was all very last minute, the cost was really high. With better preparation, I probably could've saved 20-40% on the airplane (and train) tickets. Here are the methods of transportation I used between countries: ✈️ Plane - Canada - Netherlands ✈️ Plane - Netherlands - Ireland ✈️ Plane - Ireland - England 🚆 Train - England - Netherlands ✈️ Plane - Netherlands - Canada In the Netherlands, I used buses, trains, ferries and walked In Ireland, I used a bus to and from the airport and walked once at my accommodation (at one point I walked 35 km in a day) - I did use a public bike at some point In England, I used the tube to and from the airport and also walked once at my accommodation. This summer season was notoriously difficult everywhere - so many lost luggages, delayed flights and all - my flights were constantly delayed - I'm grateful I was OneBagging as most people received their checked bags extremely late, and often lost. 🏨 ACCOMMODATIONS & LODGINGS NETHERLANDS - I have lots of family in the Netherlands so I had the privilege to have a temporary address while there - for 2 weeks, I stayed at the same apartment that was a 4 minutes walk from a ferry to Amsterdam! Jacked & Ran - Concurrent 5/3/1 & Boston Marathon training
Synopsis // A 43-year-old guy tries to run a prestigious marathon despite being out of shape, while also trying to break a 1k total for the first time. Training history // I came to lifting from a distance running background, taking up lifting in the spring of 2020 when covid shut down races around the world and I needed a new challenge. Started out with the nSuns LP, then moved on to SBS RTF. I really enjoyed both of those programs. Had some injuries and setbacks, lost some strength, tried to get back into the swing of things. S/B/D at the start of this training block was 300/260/390 for a 950lb total. Boston thru the backdoor // The Boston Marathon sets qualifying standards that are adjusted for age and gender. However, meeting those standards isn’t enough to get in. Every year more people meet the standards than there are spots in the race. Meeting the standards just gives you the privilege to submit an application. You can “qualify” for Boston and still be denied entry. In reality, you have to be a little bit faster if you want to get in. The fastest applicants in each age group are accepted until all the spots are filled. Runners refer to this as the “cut-off” or buffer on top of the actual standards. This true cut-off varies from year to year. Most years it’s only 1-2 minutes faster. Guessing this year’s cut-off is a game that runners like to play because you need to talk about something to take your mind off the monotony of running in circles for fun. You generally apply using a qualifying time from any marathon you did the year before. So for Boston 2021 you would submit your best time from some race in 2020. Because covid-19 cancelled all races in 2020, they made a rare exception and allowed people to submit a time from 2018 onward. This is where things got interesting for me. Way back in 2018 I was a 155lb skinny dude that didn’t lift and only ran. When it came time to apply for Boston in April of this year I had been lifting for about 12 months. I had bulked up to 188 lbs and had a 950 lb total. I was barely doing any running and was much much slower. But, according to the new rules, I was still allowed to submit a qualifying time from my skelly days. I felt kinda bad doing this. It was like making a Tinder profile using an old photo that doesn’t look anything like you. Instead of the typical 1-2 minute cut-off seen in most years, the cut-off for 2021 was a whopping 7 minutes and 47 seconds. The highest it’s ever been. The skinny version of me beat the standard by 9 minutes and 52 seconds, so I was in by a comfortable margin. The current version of me couldn't run more than 5 miles without stopping, let alone qualify for Boston. The pace I ran to qualify (6:52/mile [4:15/km]) was completely out of reach in my current state. I might be able to hold that pace for one mile all out on a good day. I had a long road ahead of me to get into the kind of shape where I could even finish a marathon. I owed it to myself to not half-ass the race just because I was bigger and less focused on running than I was in the past. I wanted to put in an honest effort for both sports. I still hadn’t broken a 1k total and I really wanted to hit some long-term strength training goals. At the very least I didn’t want to lose any ground with my lifts. Picking a strength training plan // Initially, I had my eyes set on General Gainz. I messaged /u/just-another-scrub because I had seen his write-up of GG in WR. He told me it was a great program, but mentioned that 5/3/1 might be a better fit for my goals. I bought 5/3/1 Forever and gave it a read. I woke up naked in a grassy field with the phrase “5S PRO??” written across my chest in someone else’s blood. After recovering I decided to re-read the book along with a copy of Beyond 531, extensive Google searches and some meditative breathing. Based on recommendations from JAS I ended up going with BBB for the first training block and Pervertor for the second block. The notion that 531 is written for athletes was appealing to me. Being able to lift 3-4 days per week was ideal for someone planning to run 4-5 days per week. I also wanted to avoid doing a full-body workout. I needed to be able to separate squat sessions from hard running sessions, as I’ve found that combo to be too much for my legs in terms of recovery. Goals // I’m a goal-oriented person so I set some goals going into this training cycle. I set a range of goals from the reasonable to the ambitious. Lifting: C goal: Don’t lose any overall strength (maintain my 950lb total) B goal: Finally hit a 1k total A goal: Hit a 1,050lb total A+ goal: Become the topic of a NoJ post Running: C goal: Finish the race / Don’t end up in the medical tent B goal: < 4:00 marathon (9:09/mile pace) A goal: < 3:50 marathon (8:46/mile pace) A+ goal: < 3:40 marathon (8:23/mile pace) Strength training // I did two training blocks. Each block was 11 weeks long and consisted of 2 leaders (3 weeks each), 1 deload week, 1 anchor (3 weeks), and 1 PR test week. I chose to lift 4x per week. Wendler says you can run the programs 3x per week. In the back of my mind I was prepared to drop to 3x per week if it became too much, but it never came to that. All of Wendler’s programs include some conditioning work. I substituted all of that with running mileage. Block one (4/26/21 - 7/18/21): The first block was 2 mesocycles of BBB and one mesocycle of 531 FSL. For BBB, I kept accessories to a minimum, hitting 0-25 reps of push & pull. Wendler says not to do single leg work with BBB, and I didn't need any convincing. For FSL, I did more accessories and increased the total reps into the 50-100 range. An outline of how I structured the cycle is here. Monday Tuesday Thursday Friday Main lift Squat Bench Deadlift OHP Accessories Arnold press, Meadows row, ab wheel Dips, pull-ups, bulgarian split squats Shrugs, Lu raises, ab wheel BB row, incline press, single leg RDLs Block one results: At the end of this first training block I did a PR test. The formula Wendler uses in his book for estimated 1RM is (weight x reps x .0333 + weight), so that’s what I’ve used below, rounding down. Squat Bench Deadlift OHP Big 3 total Start 300 250 390 175 950 End 315x2 (e1RM 335) 245x2 (e1RM 260) 380x3(e1RM 415) 165x3 (e1RM 180) 1010 Finally hit the elusive 1k total, adding 60lbs to my total. Overall, I really liked BBB. I had chest DOMS after every bench day for six straight weeks. I found 5 x 10 deadlifts to be dreadful. FSL was more sustainable, and I could see myself training that way for long periods of time. Although I got stronger, I still lowered my TM for my lifts prior to beginning the next training block based on Wendler’s recommendations. Block two (7/19/21 - 10/3/21): The second block was 3 mesocycles of Pervertor (2 leaders/1 anchor). Pervertor is the ADHD template. The main lifts use 5S Pro. But the supplemental lifts vary from week to week. It jumps from 10x5 @ FSL to 5x10 @ FSL, then 5x5 @ SSL. The anchor was even more crazy. One week you’re doing a 1x20 widowmaker set, then the next you’re doing SSL, then FSL. He really just throws the kitchen sink at you with the hopes that something in there will elicit a response. Accessory work was similar to the previous block. Block two results: At the end of the anchor template I did another PR test. Squat Bench Deadlift OHP Big 3 total Start 335 260 415 175 1010 End 315x4 (e1RM 355) 245x4 (e1RM 275) 385x3(e1RM 420) 170x3 (e1RM 185) 1050 This block ended exactly one week before race day. It was risky to do a PR test for the big 3 this close to the race, as I could have tweaked something, but curiosity got the best of me. During that one week window between wrapping up 5/3/1 and race day I did zero lifting. Running training // My most recent big race was the Houston half marathon in 2019 where I ran a 1:24 (6:24/mile [3:58/km]). Again, that was back before I was lifting. In the interim I lost a metric shit ton of fitness and gained 33 lbs of bodyweight. I do my own programming for running, so I came up with a comeback plan that would work alongside 5/3/1. I ran 4-5 days a week while lifting 4 days a week. Running consisted of one hard workout per week, one long run and the rest was just easy runs. Early on in the cycle my easy pace was in the 9:45/mile range, but by the end I was doing long runs at 8:15/mile pace. I used to do long runs at 7:15/mile pace while holding a conversation with teammates, but those days are long gone. On days where I ran and lifted I didn't do them back to back unless life circumstances required it. Typically, I would do one in the morning before work, and the second one in the evening after work, with about 10 hours in between. I found this to be sufficient recovery time. The overall layout looked like this: Sun Mon Tues Wed Thurs Fri Sat Lift Rest Squat Bench Rest Deadlift OHP Rest Run Easy 4-5 miles Rest Hard 6-7 miles Easy 4-5 miles Rest Easy 4-5 miles 16 mile long run This generally came out to 35 miles per week give or take. Historically I like to average 60-75 mpw when doing a marathon, so this was a 50% reduction in training volume to make up for all the lifting. I strained my calf about 5 weeks out from race day. It would seize up on me on occasion and was moderately uncomfortable when going up a flight of stairs. Rather than take time off, I used a combination of ibuprofen, heat and compression wraps. I dialed back the intensity of some runs but kept the overall volume the same. By the time race day rolled around it was 80% better. I tapered down my mileage the last 10 days prior to the race. I usually do a tune-up 10k race a few weeks prior to the marathon to see where things stand in terms of aerobic fitness. It helps me gauge how the training block is going and gets me into the mental mindset of racing. I didn’t do it this time because racing in the summer is terrible and I wasn’t going into this marathon shooting for a PR. Diet // I’ve always done intuitive eating, letting my training dictate my caloric intake. I kept a mental tally of my protein macros, but otherwise used my bodyweight, energy levels and performance as a metric of whether I was eating enough. My wife and daughter are both vegetarians, so family meals are generally meat-free. I would personally still eat meat if we went out for dinner. Breakfast was the only meal where I ate the same thing everyday: overnight oats with a scoop of unflavored whey protein and 5g of creatine. Otherwise, meals were highly varied as my wife and I both like to cook. I’m natural and have never taken PEDs. The only supplements I take are fish oil and creatine. I made it a goal to get 8 hours of sleep and averaged closer to 9 hour per night. My highest caloric intake was on long run day. Before every long run I had 2 Pop-tarts for breakfast (400 calories). It was an easy-to-digest sugar rush and their s’mores flavor is divine. Immediately after the long run I’d drink two 20oz Gatorades (280 calories). Then I’d hit the shower and stop at In-n-Out to get 2 double double cheeseburgers and an order of fries for lunch (1,710 calories). In the evening I’d eat dinner with the family and then go out for dessert and get a large turtle sundae (1,027 calories). That generally put me in the 4,200 calorie range for the day. The rest of the week was more scaled back with closer to 3,500 calories. My body weight at the start of the training block was 188lbs and it ended at 199lbs. Race day // The Boston marathon finish line is in Boston, but the start line is 26.2 miles away in a small town called Hopkinton, MA. You wake up at the crack of dawn, get on a bus at Boston Commons, and they ship you off. You spend the long bus ride thinking wow this is really far away and why did I pay so much money to do this to myself. They drop you off at the start line with a few thousand other runners and line you up in the order of your qualifying time, with the fastest people in the front and slowest in the back. So I was surrounded by people who were planning on running significantly faster than I was. Presumably many of these people have continued to train hard and have gotten even faster in the 1-2 year interim between their qualifying race and today. Unfortunately, I had done the exact opposite. I had to prepare myself mentally that I was going to get passed right from the start and would continue to be passed for the entire race. It’s very easy to get caught up in the crowd and adrenaline rush only to find yourself falling apart by mile 16 because you went out too fast. I had to put my ego aside and run my own race. Conditions were not ideal with temps around 65F/18C and 90% humidity, but you deal with whatever the day gives you. The course is predominantly downhill for the first half followed by some poorly placed uphills from mile 16-22. I cross the start line and took the first 5k in 23:27 (7:33/mi pace). Much faster than I had intended but adrenaline is a helluva drug and I'm bad at taking my own advice. I’m running by myself without a pack to latch onto because there’s no one around doing my pace. Despite telling myself I’m going too fast I feel ok aerobically so I just keep rolling with it, hitting the 13.1 halfway mark in 1:41:26 (7:44/mi pace). Still too fast, but the energy from the crowds is pretty wild and I felt ok. I hit the uphill section and things get real hard real fast. I’m drinking water and taking energy gels regularly to keep the engine going. I survived the uphills and hit the 24 mile / 40k mark. At this point my pace has only dipped down to 8:20/mi despite everything which is still faster than my goal pace. All of sudden, everything starts cramping up. Hamstrings and calves seize up and I know if I push any harder I’ll end up limping my way to the finish line. Only 2 miles to go and everything is shutting down. I had come too far and put too much into this training cycle to be brought down by a rogue hamstring. My pace had suddenly dropped to 9:58/mi pace and I’m in a world of hurt. I see the sign that says 1 mile to go and manage to dig deep taking that last mile at 7:30/mi pace. Official finish time: 3:29:44. Wrap-up // I added 100 lbs to my total and 11 lbs of bodyweight. While not amazing for 22 weeks of training, I’m looking at it in the context of trying to balance it with marathon training, so overall I’m still very happy with the results. There’s a lot of misconception about 5/3/1. People say it’s low volume. That’s blatantly false. Sessions would take me 90-120 minutes depending on the template. I enjoyed these templates and plan to run them again in the future. It works perfectly along side athletic endeavors. The TM is just a starting point for the program progression and its relation to 1RM doesn't really matter. Having the word "max" in the name makes you think it has some correlation with your 1RM. I lowered my TM between my first and second training block and still ended up adding an additional 40lbs to my total. I beat my A+ running goal by over 10 minutes on a hard humid course. Compared to my personal best, my overall marathon pace only dropped by about one minute per mile despite doing 50% less training and weighing 45lb more. I still don’t know how I did that. Marathon training while also trying to improve your lifts can be done. But, I’ll be honest, it’s still very very difficult. Waking up early to workout is easy. Coming home from work and doing it again sucks. All that being said, half-marathon training is less miserable and I think that could be more easily balanced with lifting. It would still require a fair amount of dedication. Up next // At the end of the day my lifts still suck. Thinking back to the post by /u/The_Fatalist on why you should stop qualifying your results, it would be easy for me to say my lifts aren’t not bad for a distance runner in his mid-forties with only 18 months of strength training under his belt. But I’m not going to do that. I’m going to keep at it and see how high I can take these lifts. I’m going to try Deep Water for beginners because /u/MythicalStrength has spoken so highly of it. Boston is my marathon swan song. I’m going to stick to the half marathon from now on. I’m running the Houston half marathon in January with some friends. We’ll see if Deep Water and half-marathon training go well together or if I will regret that decision. After Deep Water I’m going to run Simple Jack’d. If I ever get my lifts out of poverty range I may even do a powerlifting meet someday just for the experience. I don’t know if I’m going to keep trying to balance running and lifting, or if I’m just going to hang up my racing shoes, bulk to 242 and become a meat fridge. Next summer when it’s blazing hot in my garage gym I might join a gym and run one of Meadow’s bodybuilding programs just to put some meat on these bones. Gratitude // First and foremost, I have to thank /u/just-another-scrub. He was an invaluable resource and an all around stand-up dude. The guy should ghostwrite the next edition of Wendler’s book. Although he’s probably not reading this, I have to thank Spengler. He’s never spoken to me directly, but his work left a lasting impression on me. I started out in /r/fitness asking dumb questions in the daily and reading the goddamn wiki. I would not be here today without him. Also a big thanks to all the mods and regulars on WR. Posting here is anxiety-inducing because y’all are strong af and way more knowledgeable about this stuff than I am. It's highly motivating seeing the folks on this sub doing impressive stuff all time. This entire post is really just the B-movie version of /u/DadliftsnRuns deadlift & mile run post. Everyone has been super supportive and I learn new stuff all the time. Thanks for reading! submitted by /u/jaylapeche to r/weightroom [link] [comments]

Pain Regimens For Noobs
A brief guide to different treatment options I wish I knew a little more about before starting intern year. This is definitely an area I don't consider myself an expert on so I am looking forward to feedback, corrections, and suggestions for improvement! Brief overview of some of the treatment options available to you when a patient is in the hospital and complaining of pain. Tylenol (650mg q4hr prn or 1000mg q6hr prn) Max 4g/day in normal patients, max 2g/day in patients with cirrhosis / liver dysfunction I basically put every patient on prn tylenol on admission Toxicity isn't usually seen until about 10 grams of tylenol in one day Also referred to as APAP by some people (N-acetyl-para-aminophenol) - I think sticking with tylenol or acetaminophen is better: APAP: An Error Prone Abbreviation Consider scheduled doses in patients with uncontrolled pain Oral or rectal are good routes. IV is good but the hospital doesn't like you using it too much because it is expensive. NSAIDs Make sure patient is not at risk of GI bleed or has an AKI. Not commonly used for inpatients due to these risks. Ibuprofen (MOTRIN, ADVIL) - 400-600mg PO every 6-8 hrs I like 400mg because the evidence seems to show it has the same analgesic effect and as you go higher you only increase the risk of side effects. Naproxen (ALEVE) - can be dosed every 12 hours. Higher risk of GI bleed Ketorolac (TORADOL) - IV/IM forms available which can be useful especially in patients with nausea. Useful in kidney stones. Highest rate of GI bleed though debatable. 10-15mg is equivalent to 30mg in terms of pain relief. Q6h x48 hours is an appropriate trial in setting of acute pain, after that can transition to celecoxib to reduce the risk of GI bleed or renal issues (200mg BID) Indomethacin - commonly used in gout, 50mg TID during flare. For general treatment of pain, 20mg TID (I don't see this inpatient much if ever). Celecoxib (CELEBREX) - newer evidence does not show increased risk of heart attacks / ischemic cardiovascular events. May have lower risk of GI bleed (Chan, et al 2017). Might start seeing this used more going forward (although I personally have never ordered this yet). Lidocaine patch Good for musculoskeletal pain Up to 3 patches Placed and removed every 24 hours 5% form is hard to get outpatient due to insurance coverage issues but there are OTC 2% and 4% forms Capsaicin cream Some patients like it, others don't like the burning feeling Binds to nociceptors in the skin -> initial excitation and enhanced sensitivity -> followed by refractory period with reduced sensitivity and persistent desensitization, possibly due to depletion of substance P Most effective for osteoarthritis (0.025% cream), neuropathic pain (0.075% cream, not available at our hospital), and cancer-related pain Double-blind trial showing 80% of patients with osteoarthritis had reduced pain after 2 weeks: https://pubmed.ncbi.nlm.nih.gov/1954640/ Efficacious, but moderate to poor efficacy: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC404499/ High reports of burning/redness, but patients liked it and continued to take it due to decreased neuropathic pain: https://pubmed.ncbi.nlm.nih.gov/9256142/ Diclofenac gel (VOLTAREN gel) 1% gel up to 4 times daily Often used in osteoarthritis 3% gel is for actinic keratosis not for pain Low systemic absorption so avoids most of the side effects of NSAIDs Opioids Tramadol Partial mu and serotonin receptor agonist Side effect = lowered seizure threshold Falling out of favor as patients have very different responses Everyone on Reddit thinks its trash 50mg q4-6hr prn Oxycodone Commonly start with 5mg dose in patients, 2.5mg if elderly Onset in 10-30 minutes and lasts 3-6 hours Extended release version = OXYCONTIN (lasts for 12 hours) Hydromorphone (DILAUDID) The reasons patients like it is because it has a more rapid onset of analgesia which can produce more euphoria Metabolized by LIVER (don't give in liver dysfunction!) Morphine Good for air hunger, decreasing anxiety Frequently seen in patients on hospice or comfort care Has a liquid version called ROXANOL Has an extended version called MS-CONTIN Metabolized by KIDNEY (don't give in kidney dysfunction!) Meperidine (DEMEROL) Thought to be good in pancreatitis due to decreased contraction of Sphincter of Oddi Useful in patients with rigors Miscellaneous Common outpatient meds - hydrocodone-acetaminophen (NORCO), oxycodone-acetaminophen (PERCOCET, VICODIN). Can continue this when inpatient Make sure everyone on opioids has opioid overdose order set (includes naloxone for reversal if needed) Make sure everyone on opioids has a solid bowel regimen (at least miralax and senna)! Constipation common USE THE CONVERSION CHART TO SEE DOSING EQUIVALENTS When switching from one opioid you may want to consider initially dose reducing about 75% to account for differences in pharmacokinetics Neuropathic pain treatments Remember neuropathic pain vs nociceptive pain. Neuropathic pain does not respond well to opioids. Duloxetine (CYMBALTA) Venlafaxine (EFFEXOR) Gabapentin (NEURONTIN) TCAs are also used for this but very unlikely would be started inpatient, lots of side effects Methadone Used in patients who are trying to wean off opioids - long-acting IV AND PO methadone has QT prolongation which you should be wary about, get pain management involved Ketamine Consult pain pharmacy you crazy person lol Gaining popularity as an opioid-sparing pain med Side effects = emergence reactions, hallucinations Buprenorphine Also gaining popularity Partial agonist of mu receptor - meaning it acts as an agonist when there's no opioid around. But when there is opioid around it becomes an ANTAGONIST so it prevents people from doing opioids on top of their prescribed pain meds Also has reportedly a lower risk of respiratory depression compared to traditional opioids Miscellaneous Lidocaine swish & swallow - for mouth pain / ulcers Lidocaine gel (URO-JET) - for painful Foleys Warm compress (K-PAD) - for pain related to localized swelling Muscle relaxants - baclofen, cyclobenzaprine, tizanidine, etc. - they all basically work just by making you sedated and sleepy from what I learned. Doesn't really actually do that much for pain. Baclofen really should only be used for central spastic disease, it's not a great drug, also watch out for baclofen withdrawal in admitted patients (looks kind of like alcohol withdrawal, risk of seizures, etc.) Long-acting vs short-acting opioids - you shouldn't really ever use long-acting narcotics in the standard patient population. They have high abuse and overdose potential and don't really work well for most acute or chronic pain issues. Their role is really in pain related to cancer or end-of-life related pain. In cancer patients you often will see them combined with short acting in a manner similar to basal-bolus insulin regimens. Per /u/Dominus_Anulorum Don't forget to make use of consult services Pain pharmacy consult Wound care consult Post-Operative Pain Control Per u/MMOSurgeon, read more comprehensive post here For midline, subcostal, Chevron, or Mercedes incisions - thoracic epidural is the gold standard Standing tylenol is important! Toradol is an excellent choice, also order it standing Oral medications ALWAYS WORK BETTER AND LAST LONGER than IV pushes or PCAs Consider regional blocks Read the guide from Michigan for discharge to home / how many opioids to send a patient home on Formatting this on Reddit was a little difficult, message me if you want a link to a video I made on the subject or to another website where the formatting looks a little better (also has the rest of my guides on there).
