Hi! I’m a nurse practitioner. Whenever a diaper rash doesn’t clear with desitin or a zinc based cream it is likely a fungal rash. You need to put anti fungal cream on first (OTC lotrimin/ jock itch cream will work) then put the Destin on top. You need to continue using the anti fungal cream for 3 more days after the rash clears to make sure that it is gone.

Desitin diaper cream has been a life saver! A little goes a long way and it covers it. I sometimes add corn starch ontop to soak in moisture and it goes away by the next diaper change. I’ve also been giving my LO breast milk baths. But my LO has been teething hard lately and when her gums are less irritated I notice the diaper rash goes away a bit. It’s weird how the two can go together.

Lots of good ideas in here! I just wanted to suggest Desitin cream. I used aquaphor for the first few months with my first and his diaper rash kept coming back. Desitin is a much stronger heavy duty cream made for severe diaper rashes. I buy a 16oz tub of it on Amazon and it lasts a while. It is also on sale right now.

Our son had the WORST diaper rash too with open sores and bleeding. What saved us was A+D cream mixed with Desitin. The A+D cream was critical though for helping to soothe and close up the sores, so whatever you do, try adding some A+D!! It seriously was like a miracle for us and baby’s bottom was so much better.

I am venting because I feel so bad for my 15-month-old. He has had diarrhea for 3 days straight. He’s pooping liquid up to 5x a day. It’s causing a terrible diaper rash. I’ve been continuing to apply Desitin, which usually helps, but this is bad. His whole area down there is soo red and the skin is raw - it feels like leather. We took him to the doctor today. They said it’s likely just a stomach virus but with babies, it can last 4-5 days. They gave him a prescription diaper cream with antibiotic. They also said to follow the BRAT diet for a week to hopefully bind him up a bit. They also said to use wet paper towels for a couple days instead of regular wipes. They suggested soaking in warm water with baking soda which we did try but my little guy wouldn’t sit down in the tub. He is literally afraid to sit. He stands all day or lays on his stomach or side, or wants to be picked up. He’s sooo miserable. He’s been crying all all day. It’s awful watching him squirm and clench when I try to clean him up after another blow out. My husband has to hold him down while I wipe him. Okay… my daughter is almost 6 months old and has her first diaper rash. She had a little cold this week, and was congested so pooping more often than usual. Important to note-Sunday, I was out of the diaper wipe we typically use (momcozy water wipes) but I have a subscription and was expecting them Monday. I have a whole stockpile of pampers sensitive wipes, so I tried those. Monday, I noticed a little bit of irritation. I realized that the pampers wipes leave a little bit of a film behind after wiping, and I had received the correct wipes but planned on finishing out the pampers pack. As soon as I saw the irritation, I ditched them and went back to normal wipes. I applied a thin layer of desitin max strength and carried on. Since then, the rash has been minimal.. a little red but she’s been pooping more often so it didn’t seem weird. I was applying a thin layer of the desitin with every poop diaper. Last night, she was screaming so bad when it was time to go down for bed that I started just sobbing. Her diaper was changed, she was fed, I couldn’t figure out what was going on. She eventually went down and when we woke up today, I went to change her diaper and noticed the rash was a little worse/had spread. I gave her a bath, got her nice and dry, let her air out naked before applying desitin. At this point, I’d been thinking it was time to move up to size 4s for about a week now (despite everyone telling me she would be fine in threes) and I just bit the bullet and had a pack delivered so I could get her comfortable. Normally, I change her diaper every two hours. I started changing every hour or as soon as I noticed pee (she’s also fussing when she pees) and I was using water and a cloth to clean her and patting the area instead of really wiping. I’d make sure she was clean, let her air dry for a bit, and then apply the diaper cream. Well, I noticed the diaper cream was REALLY drying out the skin… to the point where it looked angrier and she seemed more sensitive. I pivoted to aquafor. I had also FaceTimed my mom friends in a panic asking if this was diaper rash because I’ve never dealt with it before, they all assured me it definitely is. I kept applying the thin layer of aquafor with each diaper change, letting the area dry well beforehand. I go to change her diaper, same process, I notice that the two angriest spots are weeping a clear liquid. Not a lot, as in not a full drop, but they looked moist even after patting dry and letting air dry. I panicked, put an insane amount of diaper cream on, and called the pediatrician. They called me back and told me to clean with water only, and to switch to a VERY thick layer of aquafor to protect the area and let her be naked with nothing on for as long as possible. I asked about possible yeast infection… she told me that I could get lotrimin or just bring my daughter in tomorrow to check. I opted to bring her in tomorrow, and asked if I should still just get the lotrimin and she said it wasn’t necessary if I was bringing her in. I’m of two minds. My daughter is REALLY uncomfortable and doesn’t tolerate Tylenol well so I don’t feel like I can count on giving that for pain relief. I don’t know if this is a yeast infection, but I feel like I should get the lotrimin and apply a thin layer just in case it IS, and pair that with the aquafor. Wondering if anyone else has dealt with this, possible bad diaper rash or yeast infection getting so bad out of nowhere. I feel so bad for her when she’s trying to sleep, I can tell she can’t get comfortable and it’s making her really upset. Hello! We just got through a horrible diaper rash caused by antibiotics my 1 yo had to take. My pediatrician gave me this recipe for "Magic diaper rash cream" which saved the day and wanted to share it with the internet in case I can help save some little butts. (This isn't medical advice as I'm not a doctor). All of this was over the counter minus the last cream which was a yeast prescription cream. I bet you could mix everything together minus the last one if you have to wait to see the pediatrician. My 3 month old baby is dealing with a rough diaper rash. He’s been on Nutramigen since 5 weeks old (severe intolerance to dairy based formula) and exclusively formula fed since birth. He’d been increasingly fussy and gassy over the last few weeks, and starting about 1.5-2 weeks ago started getting a bad rash. Initially we treated with barrier cream (triple paste). It got worse. We went to the Ped who prescribed hydrocortisone ointment. No change, went back again on Friday (writing this Sunday night) and they switched to Nystatin ointment. It is not presenting like classic yeast- it is red, splotchy, and the area around his rectum is raw and weeping clear liquid. I am currently administering the Nystatin ointment twice daily, letting it dry, adding topical barrier which was triple paste, but today I started a corn starch/Desitin paste on top of the skin. I know that corn starch can potentially feed yeast but I am DESPERATE to dry up the moist areas, and since it hasn’t been responding to Nystatin I figured I could try this. I got dry cotton wipes and have even used a stroller fan to dry the skin before putting the barrier on. Sorry for the long rant… has anyone experienced something similar and had a remedy that worked? I am looking for any possible suggestions for my poor guy. I have lots of product that I have ordered so maybe I have something on hand already and just need to start using it. I am posting this here in case there is a mom out there with a baby boy or even baby girl this may help. I tried a few different options for my baby’s diaper rash. He would get ugly angry RED rash with open sores. The open sores were especially on his scrotum and inside his leg creases So I found one product that worked well for me and then figured out why. Aquaphor Diaper Rash PASTE. It HAS TO BE the PASTE! This is because it has not only the 40% Zinc Oxide but also because it has MAGNESIUM! The other Aquaphor baby butt stuff doesn’t have it. I hadn’t been able to find anything else in the store with either. But then… I found my proof. It was hard to put the cream on the scrotal area and leg creases all the time without making a big pasty mess or him peeing himself or the combo of the 2. I found another product that worked for me which is the Boogie Diaper Irritation Gel Spray. It has hypochlorous acid AND magnesium! This was what actually cleared most of the scrotal and leg crease wounds up. So for that I didn’t even need the Zinc Oxide. Also another tip…just don’t do pampers. Those tore him up so bad. I hear they work better for girls though. I also use honest wipes and I think those help a lot too. Very good for his sensitive skin and they don’t leave his butt all moist like others but not too drying either. It’s takes quite a few for big poopies but it’s worth it. Anyways. I’m not affiliated with any of these people. I’m just excited to share this with you all. I hope it helps someone. Maybe another boy mom too who is struggling with the same thing. I was trying for a while to figure out why that paste would clear it and then I couldn’t use anything else because the rash would come right back. I finally compared all my diaper creams (Aquaphor Diaper Rash Paste, Aquaphor Diaper Rash Cream, Regular Aquaphor Baby Care, Desitin, straight petroleum jelly, Boudreaux Butt Paste, some stuff from the NICU that was special for wound care, Sudocrem \[which I actually like too but still didn’t get the job fully done\], and Nystatin) and figured out it was the magnesium. Once I tested this by trying the spray I realized it was true. Advice greatly needed!!! I have a 6 month old baby girl and she has very sensitive skin. She is prone to diaper rash so I use desitin daily defense after every diaper change preventively. Despite this she started getting a rash about a week ago and nothing is helping. She saw the dr a couple days ago and he didn’t believe it was yeast related, but prescribed keto something 2% (unsure of the name), to use as well as desitin and it’s only helped marginally. In the past changing her every 45 ish minutes and loading on diaper cream helped, but right now this rash seems resistant to that approach. My baby doesn’t seem in pain, but it is a terrible rash where some of the skin is very red and raw. Also she is prone to popping whenever her butt cheeks are spread or sometimes when I apply cream, it seems this reflex is making it harder since she frequently has poop on her. Sharing in case this helps another parent. My baby had an awful diaper rash — open skin, bleeding, super raw — that would NOT go away for 3–4 weeks. We tried everything (zinc creams, air time, frequent changes) and it would slightly get better but come back more aggressively. Desitin was not helping. We finally saw a new doctor who recommended Triad hydrophilic wound dressing (Coloplast)- it’s an orange bottle. It’s a bit pricey (~$25 on Amazon), but I didn’t care at that point. His rash cleared up in 3 days, I was so relieved. Open skin healed, redness gone. If you’re dealing with a diaper rash that won’t heal or keeps returning, try this cream. 0M 4wk old infant male with pmh diaper rash taking vitamin D supplement, topical bacitracin, 40% zinc oxide cream. Baby was born last month immediately got diaper rash. Got him on the max strength desitin and it went away. Grandma was very hard to get on board with desitin as she ate some of it and did not like the taste and put some in a small cut and felt it burn. Grandma is taking care of the baby 4/7 nights per week so getting her on board is vital. A week or so later it came back. We made a telehealth appt for him. We saw a pediatric emergency medicine MD. She said not fungal, likely friction. Give him time to air out after changes. Change more frequently. We had switch to 20% zinc oxide because it was cheaper she said go back to 40%. She said apply bacitracin under the desitin. Next day it honestly looked worse so we made an in person visit. Grandma was again very concerned that it was the desitin actually causing the diaper rash. We saw a PA at urgent care. She made the same recommendations as the MD. She said desitin unlikely to be the problem. She said switch from fancy diapers (coterie - idk how else to put it) to super absorbent like pampers to make sure his butt is very dry all the time. Also we didn’t have bacitracin that first night so I put triple antibiotic and she said no good get bacitracin and we did. Grandma is still very concerned that the desitin is causing the diaper rash. Grandma had a fight with my wife in another language. Grandma then came to me for help. She shared with me that she felt the bacitracin that the Md/pa recommended was not strong enough for our baby’s diaper rash. She recommended gram - coverage for the diaper rash. She informed me that she had begun to apply topical 2% mupirocin to the diaper rash for the past two days and that the diaper rash had improved (also coinciding with changes in everything else mentioned above). I was the wrong person to come to for help with this. Wife didn’t know. So I’m very upset and I took the cream away before she could use it again. Help! It’s been, fuck I don’t even know, like 4 months? Of just chronic diaper rash for my poor kiddo. Several doc visits and scripts and they just say he’s unfortunately got sensitive skin and will likely suffer like this until he’s potty trained (he’s only 14 months (13 adjusted for prematurity)) we can’t live like this, we plan to cross our fingers and hope for the best with early potty training but man this could drag on! EDIT: We finally started seeing improvement with our diaper rash!! For anyone who has found this post later down the line, for us it took a combination of triple paste maximum strength (red tube), whoopsie dry wipes instead of wet wipes, Huggies diapers (sized up), and a twice a day application of hydrocortisone 1%. I sit there and hold a desktop fan up to his booty to dry the hydrocortisone on for 5-10 minutes before layering triple paste over it. Also a little bit after the improvement started, we gave him a bath with 150mls of breastmilk added to the water. Unsure if it helped, but it didn’t hurt! ——————————————— Hi everyone! My son will be 4 weeks old tomorrow and has lived about 2 days without a diaper rash. This thing is constant and will not go away! She came home with a diaper rash that started on her anus, little sores. And then it just has gotten worse and worse and worse as I actively did everything to treat it. I’ve tried everything. I’ve read every Reddit thread. I googled and chatgbt-Ed it. I took her to the pediatrician, then to the dermatologist, I’ve changed her diaper brand (we used pampers pure, Huggies sensitive, and now on Millie moon). She airs out, I dry her out with a fan, i don’t use any wipes, i used the steroid cream, when that didn’t work, they gave me the yeast cream to try (that’s what we’re currently trying), and I’ve used every zinc oxide cream you can think of. Could she be having a reaction to the diaper cream? Like it’s been 5 weeks now and her skin looks horrible. There’s a large deep sore. And when I put the diaper cream on she screams. Should I stop all use and continue the yeast cream and just put some plain Vaseline or Aquifer? I feel so so bad for her, her butts so raw and it’s growing worse like where the cream sits. EDIT: I took her to her pediatrician today. She said to stop use of everything other than the anti fungal and to create my own paste using 1 bottle of extra strength desitin, 1 tube of Bacitracin (just not neosporin), and 30 ml of mylanta. My wife is in terrible pain and is starting to feel hopeless. She has eliminated everything from her diet that is not whole foods. Eating only meat, vegetables and fruit for over a year. We've both been researching ways to remedy this condition and nothing seems to work. We have Desitin in our home for our baby, but she's afraid to use it. My 4 week old has had a bad diaper rash for over a week now. Like open sore, raw perianal lesions. I have spent hours combing the internet for things to try and we went to the pediatrician earlier this week and got some prescription creams too. So far we have tried: -no wipes, instead spritzing with warm water and dabbing with a cotton round -loose diaper -more frequent changes -hairdryer -aquaphor -pure petroleum jelly -desitin (store brand, the blue tube) -calmoseptine -baking soda bath daily -diaper free time (my husband went as far as building a box that holds a puppy pad like a surgical drape so he can lay in there and not pee all over himself) -lotrimin -when lotrimin didn't work, pediatrician gave us Nystatin and muproicin 3x daily, we're on day 4 with very little to no improvement, but it's not getting too terribly worse The only things we haven't tried are switching diaper brands (but if it was the diaper brand, wouldn't it be all over and not just his perianal area?) and a major diet change. We were exclusively pumping, but my supply is taking a nosedive. The stress of this and constantly changing diapers means I'm pumping less too. He's currently getting half breast milk and half gentlease. Totally fine with switching to full formula like a soy or hypoallergenic if it's a diet thing. What am I missing? Do I just need to stay the course and give it time to heal? Pediatrician nurse this afternoon said to wait until Monday and if it's still not getting better to call again. What more can they do or prescribe? His cries during diaper changes absolutely kill me and I spend all day trying not to cry from the guilt I feel that I haven't made it better. I saw a few posts this week from parents struggling with diaper rash, and I wanted to share what worked for my family because it was a GAME CHANGER and I’ve never seen it discussed anywhere. Around 4 weeks old my kid got absolutely horrible diaper rash. Like her buttcheeks were chapped and raw it was so awful. It came with a yeast infection that required a prescription (please look up what a yeast rash looks like cuz sometimes that’s the only answer you’ll need), but even when we cleared up the yeast the rash and damage from the rash was so bad it didn’t solve the problem. My mom, thank god, is a 40 year NICU nurse. She gave me the tools that cleared this rash up in a week and have stopped any future rashes right in their tracks. And now I pass along this golden advice to you: 1) Desitin stings babies sometimes. If you notice your baby crying when using, stop using it. You want something with 40% Zinc Oxide during the rash, we used max strength Butt Paste but there’s a few options. 2) At the first sign of a rash, stop using wipes and switch to soft, clean cloths and water. If you can, just rinse poops off directly in the sink to minimize wiping as much as possible. 3) THIS IS THE HIDDEN SECRET OF THE NICU: Vashe Wash. Squirt that miracle fluid on the rash every single diaper change. Be liberal with its application. Mom said you could dab it off after or just leave it, we always left it and it worked a treat. 3) Stroller fan to dry them off before putting cream on (or any small handheld fan will do). They’ll probably pee from the cold air so make sure you have a cloth or diaper under them when you do this lol but it’s critical they’re dry before applying cream and this again limits wiping/dabbing. In terms of actual diapers, avoid anything scented if you can and diapers with multiple liners like Pampers as they tend to stick to the skin when wet (that being said, some babies will only tolerate Pampers so I’m not trying to demonize any one brand here. Basically, whatever you’ve tried lined or unlined, do the opposite). I hope this advice helps out another family as much as it’s helped ours! Hello! My son has slept through the night since about 6 weeks. He’s now 9 months old. We have the problem of never ending diaper rashes. I change him the second he poops but if he ever poops after I put him to bed at night then he’s sitting in it for many hours. The pediatrician has me use anti fungal cream, Desitin, and baking soda soaks. Is there anything else that helps? Product recommendations? Is there anything that would help prevent diaper rashes? I’m currently snuggling an upset boy and his butt is bright red. Baby is 10 weeks old and we are on our 4th round of antifungal treatment. Rash clears temporarily but comes back a few days after stopping antifungal. I think the root of the problem is wet farts and unable to keep a good barrier around his anus. We’ve tried so many creams (calmoseptine, aquaphor ointment, aquaphor 40% zinc, triple paste, triple paste max, desitin max, desitin daily defense, Burt’s bees) and I’m probably missing a few. NOTHING sticks to skin around anus after a fart. It’s like the fart blows/pushes the cream away. I’ve heard good things about calmoseptine sticking, but I tried it and it did not work. I layer aquaphor ointment on top of whatever zinc product im using. Again cream will stay on butt cheeks but his anus keeps getting irritated and yeast comes back - because NO cream will stay on anus after baby farts. Any advice on best diaper cream that will stick? Every so often I see people posting about their babies’ bad diaper rashes that seem to be resistant to the normal intervention of diaper cream and frequent changes. My daughter had such a bad diaper rash in her first few weeks of life that it actually developed sores. I was beside myself. I was putting diaper rash cream on around the clock, trying to air it out, everything. The pediatrician gave us this protocol to cure it and it worked like a charm at the time, so I thought I’d share: - Give baby aveeno oatmeal baths (no soap) once a day while rash is present. - Make sure they are very, very dry before applying diaper cream (so if you habitually wipe, then apply cream immediately after, you’re likely trapping moisture). We would pat (not rub) her dry after wiping with a soft cotton cloth when we couldn’t give her enough “air time.” - Make a “potion” using equal parts calmoseptine, purple desitin, and either triple paste max or Boudreaux’s max. Calmoseptine is a pink OTC ointment you can usually find in the adult diapering aisle of your pharmacy. Mix these together really well (we stored it in a small airtight Tupperware container and keep it around for any future rashiness). Every time baby has had a diaper change and is thoroughly dry, you apply this “potion” like you’re frosting a cupcake— a really, really thick layer to the rash. It soothed her horrific rash within a day or so. Within 3-4 days, it was totally gone. This may seem like a long time, but trust me, it was one of the worst diaper rashes I’d ever seen and the pediatrician agreed. FTM of a 7 week old and have been at war with a diaper rash for the last two weeks. It started out appearing very red and sunburned with areas of raw skin around the pressure points of his butt. I jumped into action immediately and aired him out, cleaned only with water, dried the area and applied liberal amounts of diaper cream with each change. I’ve tried desitin, triple paste both original and max, and aquafor. Went up a diaper size to reduce friction and tried to switch from pampers to Huggies and eventually to pampers pure. The overall redness went away but the raw skin remained. Went to pediatrician who prescribed Nystatin powder to apply first to have the diaper cream stick. That started to help and the raw area decreased but the raw spots although smaller have essentially been unchanged in the last week. It appears like small cuts/ open blisters and dots of raw skin localized around his anus. We called the pediatrician again and she advised adding neosporin to the mix. I’ve been doing that for three days no change whatsoever. My son seems pretty chill and unphased but it still appears so raw and I feel so helpless and defeated. Any wisdom or support appreciated, especially if anyone has had a similar experience. ETA: thank you all for the suggestions, I’ll keep the post updated with any progress and hopefully share what eventually works! Update! I am so incredibly grateful for this community and for all who took the time out to leave their suggestions and advice! I can’t believe it, but my son’s open raw rash is almost entirely healed, and It’s been less than two days! I’m so, SO relieved. Here’s what eventually worked: After reading through every one of your comments I started to think about what the cause of the rash might be. Since the anti fungal powder didn’t completely clear it and the neosporin didn’t work I felt maybe it was more of a skin breakdown from his acidic poop issue so I didn’t immediately pursue stronger or alternative anti fungal creams or prescriptions. I definitely felt his poops were frequent (although pediatrician said the frequency was normal) and occasionally more liquidy, so I figured I’d focus on reducing irritation through a change in his formula (and thus his poops) and both protecting and healing the open skin. 1) switched formula from milk based to an allergy friendly formula non cows milk blend with probiotics. I kid you not, in the last day he’s gone poop half the frequency he did on the other formula and his poops are uniformly solid, liquid free and these solids are easy to clean off his butt. Before he pooped solids but it was often mixed with brown liquid and he would “shart” the liquid often too between diaper changes too. I had no idea this was something that could be lessened with a formula switch. 2) Tried a new cream that some of you suggested: PINXAV for the win!!! OMG. After one application I started to see the difference. I think this worked because it’s advertised not just as a barrier cream but a healing ointment for bedsores and other open skin issues which is what my son had. It also contains an antacid ingredient so I didn’t have to buy Maalox or mylanta separately to try. His open skin is almost completely closed, it’s simply amazing stuff. In addition I put baking soda in his bath, kept the bigger diaper size, and aired him out after changes with a small handheld fan. Of course every situation will be different, but I’m so happy we were able to find a solution to this issue finally. Thank you all again!!!! Update again: sadly, two days after the prior update the rash began to creep back. Was referred to dermatologist who said it’s erosive dermatitis (to me looks like Jaquets dermatitis if you look that up) and prescribed a steroid and yeast mixture cream. Hoping this helps. Our LO (3 weeks) has had a diaper rash for a little over 2 weeks. We have tried everything. It’s been red and raw for two weeks as well, missing skin, bleeding. He’s seen his pediatrician 3x and has an appt next with derm. He’s breastfed and poops constantly - every diaper change and in between. It’s not diaper brand related, per MD it’s just his poop. We’ve tried, triple paste, desitin max, aquaphor, Vaseline, mupirocin, a compounded ointment with hydrocortisone and cholestyramine that seemed to make it worse, and finally now we’re on nystatin and triple paste, using liquid bandage in between. We’ve been letting him air dry about 3 hours a day and doing daily baking soda soaks. We don’t wipe, we use warm water… My question is, why when we open the diaper is no diaper cream left anywhere? Everyone says “just add to it, don’t wipe off the remainder”, but there is nothing. We lather it on so thick, let it dry for a little. What are we doing wrong for there to be none on there? Is he just pooping too much? Any help/ideas welcomed! We’re desperate. UPDATE: if anyone is searching for posts like this in the future like I was… with the raw red skin that really is no longer a rash but just broken skin that creams/ointments don’t adhere to or work for - DO THE CRUSTING METHOD. It’s used for wound care where you put stoma powder, spray with no string barrier spray, let dry, repeat 2 more times for a total of 3 layers. Then cover with thick layer of A+D. We’re not completely healed yet but been doing it for 72 hours and half of the raw lesions have healed. It’s been a lifesaver. I work in healthcare and thought okay I’ll try this as a last resort because I was tired of spending $$ on creams that every provider recommended as their own personal fix. Hi moms and dads. Hoping I can get some sound advice, aside from my babys doctor just telling me to "keep doing what you're doing" which means apply hydrocortisone cream every time there's a flare up. My baby girl is 10 weeks old. The initial diaper rash actually started in the hospital because she was in the NICU for a few days with jaundice. It was just a few red dots close to her bum hole. Once we got her home, I kept putting the zinc oxide cream on that they sent me home with. The rash seemed to go away but I kept the cream going as preventative. Then I decided I wanted to go more natural so I got the hello bello diaper cream. Terrible stuff actually. It's liquid and gritty. That's when she started getting a more legit rash that was inside her crack and was extending outwards. So I quickly started using desitin which I had researched was pretty good. I also put some breast milk on which I heard could help. The rash didn't get any better and every day got more red and reached a wider surface area. Then one day it just absolutely exploded and her bum was fire engine red and it looked like an upside down spade shape...Basically all the areas where her bum would make contact with diaper, or any surface, if she were laying on her back. I called the after hours doctor cause it was a weekend and they said it was yeast and prescribed hydrocortisone + clotrimazole. I started the regimen twice a day as prescribed and continued applying zinc based cream on top. At this point it was desitin and sudocrem. Also doing lots of diaper free time. It really started to improve on the butt cheeks but then inside her crack it got all weepy and raw. It was awful. I went back to the doctor and they sent us to the specific pediatric care center at the hospital my doctor is affiliated with. Now I must add that my baby poos A LOT. She is breastfed and so her poos are extremely watery and very frequent. Between larger poos there is constant poo juice seeping out her bum and she sharts any time she coughs or sneezes. So clearly a big reason for the rash is all the poo. The pediatrician at the hospital said that there's nothing wrong with her and unfortunately she's just on the bad end of the poo spectrum and I'll just have to manage it. He suggested getting stoma powder to mix with sudocrem to make an effective barrier and also he said I could try ihle's paste. I got both. The stoma powder mixed with the sudocrem didn't stick very well, I found the ihle's paste had better sticking power. So I started applying tons of ihle's paste and doing hours of diaper free time. While still applying the hydrocortisone clotrimazole treatment until the rash went away. Well that was a month ago and while the rash did improve, it continues to come back. It gets better and then just flares up again all over her butt cheeks in that spade shape and also her crack is very red and raw again. Yesterday and today the skin around her bum hole is screaming red. I apply the hydrocortisone trt whenever it flares and it calms it down. I always apply tons of ihle's paste and I've even started patting the stoma powder on first to get rid of any moisture and to help the paste stick better. One problem I have is during diaper changes, of course I don't wipe all the old paste off cause that will irritate her skin, I know to just apply new stuff on top. But the old layer of paste is moist and sticky and I'm worried that's gonna trap moisture. So I add stoma powder to dry it up. I don't know if this is a thing but I don't know how to manage the old remaining paste being sticky and damp before you add new cream on top. I use warm water and cloth, no wipes. I blow-dry her bum until all the skin is dry (aside from the skin under the old paste because it just stays sticky and damp). I'm even just rinsing holding her in the shower with the wand to avoid further friction and irritation. I use disposable diapers that are chemical free. Honest brand, hello bello, babyganics, pampers pure. I sized up diapers so she's wearing diapers way too big but I'll risk the chance of leaks just to give more airflow. And hours and hours of diaper free time. I've been doing oatmeal and baking soda baths. I'm at a loss. I've done my research and read countless posts like this one. I'm reaching out now to see if anyone has any idea that I might be missing. Thanks for any help or insight! Update!! 17/12/2021 Ok, thanks to everyone for all the advice, tips, tricks, support, and encouragement. So far, I have seen some huge improvements over the past 4 days. And the kicker?? Cornstarch. I had a number of comments to try cornstarch to keep things dry. Although controversial, due to potential for inhaling it, I decided to give it a try cause I'm desperate. Low and behold, after 3 days, her bum looks incredible. The skin looks healthy and happy. I've been adding it on top of zinc cream. I pack it onto the cream itself to lock it in so it won't come off her bum and absorb into the diaper and then I add approx 2 tbsp and sprinkle it in the diaper itself. I think this is key cause it keeps her DRY. I think with the acidic poop (working on my diet to help with that one) and the continuous moisture cause her poos are so liquid and there's constant squirys coming out, that the cornstarch reduces this moisture and the rash has finally been able to heal. Her bum has never looked this good since she was 1 week old. So, now I'm kinda freaking out cause if this is what works I want to continue using it. My question is, for those of you who have also had success with cornstarch, how you apply it safely and ensure baby doesn't inhale it? Sometimes it gets everywhere. It's all over my one hand once I'm done and then I have to quickly seal up the diaper. There's cornstarch all over my changetable. I keep everything away from her face of course, but I'm worried if there's still times when it might get airborne. For example, seeing as I'm not used to the powder yet, yesterday I went to change her diaper and had to use the blow dryer to dry the part of her bum that I had cleaned. I forgot that I had cornstarch in the diaper and it blew up in the air!! I freaked out and put my hand over her nose as fast I could and ran out of the room with her. I feel like a complete moron and spent the evening googling what happens if baby inhales cornstarch. Im assuming she didn't inhale any cause she never sneezed or coughed. But maybe she did? Have I already caused irreversible damage??? Has anyone been in my position and stupidly had some powder go up in the air around baby? How do you apply cornstarch to your LO?
