Lab results are now posted in MyChart. Most are in the middle of their reference ranges; a few flagged "ABNORMAL" are 1 point outside. I am still low in sodium and chloride (I rarely salt food) but potassium is OK. Blood glucose is above the reference range but lower than it has ever been before, and A1C is 5.8.

Can hidden MyChart notes be accessed with a full medical record release? I took care of a baby a few weeks ago. Mom with substance abuse, came in several days in a row slurring her words, dozing off, repeatedly asking questions and forgetting. I put in a hidden note and CPS (already involved) visited them and told them it was because of a nurse’s note. CPS did not take custody of her or her older sister. Parents haven’t been back to NICU since, so no one else has reported any behaviors like this, only me. Now dad is repeatedly calling the unit trying to track down who wrote the note. He says he “just wants to talk” but I worry about retaliation. He talked to the manager and she didn’t share my information. But I’ve read online that they can read the “hidden” EPIC notes if they request a full release. Does anyone know if that’s true? Visits, results, and paperwork are tedious enough and then when you add patient advice requests to the mix it will one day drive me to quit. Question about the visit? No problem. Illness anxiety + ChatGPT? No. New problems? No. “Just schedule a visit” is great advice if I wasn’t booking 6 months out. No one died before MyChart and no one will die after MyChart. My 15 year old can’t request her own access, she can’t consent for me to have access, and they won’t give me access…meaning that I have to wait for the physically mailed letter, days for the doctor to call us with the results (which they suck at doing in a timely fashion), or physically go and request the paper copies in person…because I’m entitled to all that as her parent, just not online access. And she’s not entitled to any of it being a minor, including online access. She had a culture done and it took them 4 days to let us know they needed to change her antibiotic because I couldn’t get the results in a timely fashion. hi, this has been just absolutely marinating in my mind since it happened yesterday and I really think most of you will get a kick out of it (albeit a sad reality). sooooo I have a variety of medical background; i have worked in pharmacies, admin, EHR /PMS software. I am also mid 30s so I deal with my own health issues and my parents have their issues, yadda yadda. my dad actually had some trauma during childbirth that resulted in a "mild" cerebral palsy that he had to do physical therapy to overcome back in the 70s and eventually he was able to have "a normal life" with sports and anything he wanted to do. unfortunately he also was a product of his time and an abusive father who insisted he was "fine" and limited any neuro care/research. so my dad goes on to live "a normal life" and honestly when I am born and growing up, I do not see my dad as disabled. he doesnt have a regular neuro or PT/OT/etc. he does have balance issues and speech pattern issues but in my mind , thats just how my dad is. fast forward to this year and my dad has been being seen at the Cleveland clinic after his multiple covid instances have stuck with him into long covid which has exacerbated his CP symptoms and led to much more testing (and falls and the inability to work resulting in homelessness but that is neither here nor there; he is currently set to be seen in disability court in march) so fast forward to this week and my dad had a jaunt of being admitted for week long testing for seizure activity/stroke activity at the Cleveland clinic. they have also discovered he has ataxic CP/ataxia (my daughter will be having a brain MRI soon to determine if her cerebellum is healthy or if she has a genetic ataxia as she has just started walking now at 25 months with balance issues remaining) now where the story comes together: he calls me yesterday and says "I finally have a diagnosis. I have liver cancer." and I am dumbfounded and distraught and just incredulous bc this man has had so much testing in so many states over the past 20 months and it is just improbable to me that they just now catch liver cancer. so i immediately want to see his chart and I am on hipaa, I have him on the phone, he consents to make me a chart proxy and I can go see. so hes telling me about his life expectancy and im asking all the nerdy little questions and he reveals that he hasn't met with his provider to discuss the results yet. I say "where did you get the diagnosis" he says "its on mychart, it says cerebral palsy HCC"......... so my dad was looking at his chart notes and saw HCC next to his CP/seizures/diabetes/etc diagnoses and googled it. saw it was an acronym for hepatocellular carcinoma. called all his family and told them he had liver cancer. yeah. so i'm like "dad. this means hierarchical condition category. for coding. you dont have liver cancer. liver cancer is no where in here as a diagnosis." he had already told everyone. sooooooo mychart, maybe we put some sort of blurb or note indicating what coding acronyms mean so people like me dont have mini heart attacks when their dad googles stuff lmao edit: in my defense of this long ramble, I am on cough medicine 💊 🤧 tl;dr : my dad saw HCC in his mychart and thought HCC meant he had liver cancer bc he googled the acronym. told us all he had liver cancer. the chart determined that was incorrect and it was HCC attached to a diagnosis as in HCC (Hierarchical Condition Category). tell your similar lost in translation mychart stories below for catharsis ❤️ submitted by /u/stardustyjohnson to r/FamilyMedicine [link] [comments]

I understand that UT Health, Houston Methodist etc are in the HIE/ EPIC Everywhere. Does it mean patient's X-rays, photos etc are on the chart and can be seen by any provider who is on HIE/ EPIC? Not only have I had to beg for the correct help for days, scared out of my mind, heart rate of 210 and blood pressure 150 over 110, these assholes put that I am in here for OPIOID ABUSE AND WITHDRAWAL!!! I have my pain meds at home and have offered to bring them in to show any and everyone that I HAVE my pills. I am not here over opioids. Holy shit. I truly think a lot of doctors either a) hate us or b) have zero ability to empathize / understand basic shit. i've never gone to a rehab center or been asked to go, i've never had any issues w my pain doc. Also - screenshot says I came on October 2 because I came Sept 29 and left Oct 30 AMA because they refused to treat me yet wanted me to stay and be treated - w h a t - I came back on the 2nd, was immediately admitted by a sincerely kind doctor who set up IV everything (nausea, some sort of pepcid, allergy, antibiotics, pain, anxiety meds) then woke up very abruptly this morning to the same doctor I left AMA due to being on my case again and all of my help being discontinued, a rug ripped out from under me. That's when I had to beg for my life because I cannot swallow meds rn they come right back up *violently* and read out loud a 4 page fucking dissertation on why I deserve care, which felt humiliating. Supposedly I have a much kinder doctor tomorrow. Let's pray. Also, my nurse says that on her chart it says I am here for acute pyelonephritis, fevers, and allergic reaction. So I am really confused. Sent in a request to get this off my record, filled in a record amendment and sent to patient advocate. I had a question about a temporary medication I was taking and sent a message via MyChart. The message was only regarding the medication (no other health questions were asked). I received my EOB and was charged a full $50 co-pay like when I go in person for a visit or have a full video visit. When I looked online, I see in general messaging costs listed as much lower than a visit. Does this mean my insurance doesn’t differentiate a full visit from a brief question in a message? If I had known, I would’ve scheduled an online telehealth visit instead. I’ve had a lot of medical costs this year and another random $50 stings. A few days ago, my coworker noticed their ex partner was down in our ED; coworker was freaking out and decided that they had to know what was wrong, so they opened up their ex partner’s chart- except, just kidding, they had one of our other coworkers do it “so I don’t get caught.” They then proceeded to tell everyone in our office what they were reading about in the chart, in detail, including their ex partner’s entire mental health history. Their excuse of “I was so worried” quickly falls apart since they also started saying “oh it makes sense why they acted x way when we were dating…” I reported it immediately to my supervisor, and it’s not exactly a secret who could have reported them, since our office only seats about five people. Now that coworker is treating me harshly, giving the bare minimum responses when I ask about our patients together, etc. I don’t even think our supervisor did anything beyond a sit down meeting saying “this was a naughty thing to do”. The coworker who helped access the chart was given a ‘here is what HIPPA is’ module and that was it. Did I do the right thing by reporting my coworker? Should I also report this to HHS Gov? I don’t think the patient was ever informed that this all occurred- should they have been? My supervisor told me “don’t talk about this with anyone else,” and I want to hope that means they’re investigating it, but I worry that is them trying to sweep it under the rug. It’s really stressful, anxiety invoking, and enraging all at the same time. I have even been better about making patients come in for appointments, but even still, the amount of messages is draining. Especially the ones who send 5 messages in a row. Then there’s people who demand to speak with me as though I’m not working the whole day and just sitting on my ass. And I already know the things to do to cut down on mychart messages or get people to come in, but the sheer volume is the problem! Why do they give access to test results before the dr has a chance to review and talk to you about it!!! My son saw the immunologist Friday and they are running several blood tests. I am currently a third year medical student so I don’t have a lot of experience from the physician/provider side of MyChart but from what I have seen, it doesn’t seem as useful as advertised. One hand it seems like a good idea for patients to have access to all their medical data, especially when they have to go to a new doctor for whatever reason. But from what I have seen, some patients can get panicked whenever they don’t understand the results (which is often). Every attending I have worked with so far has explained that just because a number is listed in red doesn’t mean it’s something bad. They tell everyone that if there is something they are worried about, they will call. But it never fails that some random value will be off and patients will freak. I get why they freak out, it’s scary when you don’t have all the context we have but there has to be a better way to handle this kind of stuff (I don’t see MyChart going away) that not only reduces patient anxiety but also cut down on frivolous MyChart messages. Also this may be a smaller thing but every now and then I’ll be taking a history and the patient will get annoyed and just say “it’s in my MyChart”. The number of times patients read something in a daily progress note and don’t understand it and google it is skyrocketing. One got in an argument recently due to upstaging. T3N0 lung is IIb but I just wrote upstage to T3. Family read it and was convinced stage III and accused of lying. Radiology reads freak people out. It really needs to stop. Ive resorted to basically making patients wait a day or so for a reply (unless its something that cant wait) and trying to schedule them. Most times replying to patients just creates a whole pile of dialogue and I get irritated.
